Dyspnea

I have been experiencing difficulty breathing since the second year of junior high school, but it wasn't very frequent until recently.
Now, I often have trouble breathing, especially in the classroom or at night, and I find myself needing to take deep breaths without feeling like I'm getting enough air.
I often start to wheeze after walking just a few steps, which makes me hesitant to participate in physical education classes.
My heart rate frequently exceeds 100 beats per minute.
I previously underwent X-rays and an electrocardiogram, both of which were normal, but I still struggle with breathing.
The pediatrician (since I am not yet 18, I cannot see a pulmonologist) suggested that I visit a psychiatrist.
The doctor mentioned that I seem very anxious, but I don't feel particularly anxious myself.
I would like to know what kind of problem this might be and how it can be treated.

Dr. Jian Shuntian reply Pulmonology

Hello: There is also a pediatric pulmonology subspecialty, or you can consult a pulmonologist; some hospitals may accept this.
You may also consider seeing a cardiologist.
If everything is fine, you can consult a psychosomatic medicine specialist (i.e., psychiatrist), as stress can also be a contributing factor.
Thank you.
Sincerely, Jian Shun-Tian.

Breathing difficulties can be a complex issue, especially when they have been present for an extended period, as in your case since middle school. It’s important to understand that breathing difficulties can stem from a variety of causes, including physical, psychological, and environmental factors. Given your symptoms and the medical evaluations you've undergone, let's explore some potential causes and treatment options.

Potential Causes of Breathing Difficulties
1. Asthma: Given your history of breathing difficulties, asthma could be a significant factor. Asthma is characterized by inflammation and narrowing of the airways, leading to symptoms such as wheezing, coughing, chest tightness, and shortness of breath. It can be triggered by allergens, exercise, cold air, or stress.

2. Anxiety and Panic Disorders: Your mention of feeling nervous and the suggestion from your pediatrician to consult a psychiatrist indicate that anxiety could be contributing to your symptoms. Anxiety can manifest physically, leading to hyperventilation, a sensation of not getting enough air, and increased heart rate. Even if you do not feel particularly anxious, your body may be reacting to stressors in a way that affects your breathing.

3. Allergic Reactions: Allergies can cause respiratory symptoms, including nasal congestion and difficulty breathing. If you have allergies, they could be exacerbating your breathing difficulties, especially in environments like classrooms where allergens may be present.

4. Respiratory Infections or Conditions: While you have had normal X-rays and ECGs, it’s still possible that a mild respiratory infection or condition could be affecting your breathing. Conditions like bronchitis or even chronic sinusitis can lead to persistent symptoms.

5. Obesity or Physical Deconditioning: If you have experienced weight gain or reduced physical activity, this can also contribute to breathing difficulties. Being out of shape can make physical exertion feel more challenging, leading to shortness of breath.

Treatment Options
1. Consult a Specialist: Since you are under 18, it’s essential to work closely with a pediatrician who can refer you to a pediatric pulmonologist or an allergist. They can conduct specific tests, such as spirometry, to assess your lung function and determine if asthma or other respiratory conditions are present.

2. Psychological Support: If anxiety is suspected, consider working with a mental health professional. Cognitive-behavioral therapy (CBT) can be particularly effective in managing anxiety and panic symptoms. Learning relaxation techniques, such as deep breathing exercises, can also help you manage your symptoms when they occur.

3. Medication: If asthma is diagnosed, your doctor may prescribe inhalers or other medications to help manage your symptoms. If anxiety is a significant factor, medications such as selective serotonin reuptake inhibitors (SSRIs) or anti-anxiety medications may be considered.

4. Lifestyle Modifications: Engaging in regular physical activity, maintaining a healthy diet, and ensuring adequate sleep can improve overall health and potentially reduce symptoms. If you have allergies, identifying and avoiding triggers can also be beneficial.

5. Education and Awareness: Understanding your condition is crucial. Keeping a symptom diary can help you and your healthcare provider identify patterns and triggers related to your breathing difficulties.

Conclusion
Breathing difficulties can arise from a myriad of factors, and it’s essential to approach the issue holistically. Given your history and the complexity of your symptoms, a multi-faceted approach involving medical evaluation, psychological support, and lifestyle changes may be necessary. Don’t hesitate to seek further evaluations and advocate for your health until you find a solution that works for you. Remember, you are not alone in this, and with the right support, you can manage your symptoms effectively.
