Pain below the right rib cage?
Hello, doctor.
Yesterday, I was suddenly awakened by pain under my right rib cage while sleeping (this has never happened before).
The pain lasted for about 20 seconds, and after it eased, I fell back asleep.
However, upon waking up, I noticed that the pain was still there.
It hurts when I breathe and when I stretch that area, but pressing on it doesn't hurt much.
When I'm not breathing deeply or stretching, I don't feel anything.
The pain feels somewhat like a cramp, but it has persisted for too long.
The location is right under the rib cage.
Since I'm currently abroad and it's inconvenient to see a doctor, I would like to ask what symptoms this might indicate and how long I should wait before seeing a doctor.
Thank you.

Hello! If it's musculoskeletal pain, that's manageable, but if it's gallbladder colic or acute cholecystitis, you should see a doctor as soon as possible, at least for an ultrasound! Changhua Hospital cares about you!

More Info
Right rib pain can be concerning, especially when it occurs suddenly and disrupts your sleep. Based on your description, it seems like you experienced a sharp pain under your right rib that lasted for about 20 seconds, followed by persistent discomfort that worsens with deep breathing or stretching. This type of pain can have various causes, and understanding the symptoms can help determine when to seek medical attention.
Possible Causes of Right Rib Pain
1. Muscle Strain: One of the most common causes of rib pain is a muscle strain. This can occur from sudden movements, awkward sleeping positions, or overexertion. If you were in an unusual position while sleeping, it could have led to a strain in the intercostal muscles (the muscles between the ribs).
2. Costochondritis: This is an inflammation of the cartilage that connects a rib to the sternum. It can cause localized pain that may worsen with movement or deep breathing.
3. Rib Fracture: Although you mentioned that this pain is different from any previous experiences, a fracture can occur from trauma, even minor incidents. Rib fractures can cause sharp pain, especially when breathing deeply or coughing.
4. Gallbladder Issues: Pain in the right upper abdomen can sometimes radiate to the rib area. Conditions like gallstones or cholecystitis can cause discomfort that may be mistaken for rib pain.
5. Liver Problems: The liver is located in the right upper quadrant of the abdomen, and issues such as hepatitis or liver enlargement can cause pain that feels like it’s coming from the ribs.
6. Pleurisy or Pneumonia: Inflammation of the lining of the lungs (pleurisy) or an infection (pneumonia) can cause pain that worsens with breathing. This is usually accompanied by other symptoms like cough, fever, or shortness of breath.
When to Seek Medical Help
Given that your pain persists and is exacerbated by breathing or stretching, it’s important to monitor your symptoms closely. Here are some guidelines on when to seek medical attention:
- Persistent Pain: If the pain lasts more than a few days without improvement, it’s advisable to consult a healthcare provider.
- Severe Pain: If the pain becomes severe or unbearable, seek immediate medical attention.
- Accompanying Symptoms: If you experience additional symptoms such as fever, difficulty breathing, chest pain, or jaundice (yellowing of the skin or eyes), you should seek medical help promptly.
- Impact on Daily Activities: If the pain interferes with your ability to perform daily activities or affects your sleep, it’s a good idea to get evaluated.
Conclusion
While it may be tempting to wait and see if the pain resolves on its own, it’s important to listen to your body. If the discomfort persists or worsens, or if you develop any concerning symptoms, seeking medical attention is crucial. Since you mentioned being abroad and finding it inconvenient to see a doctor, consider telemedicine options if available, as they can provide guidance and help you determine the next steps in your care. Remember, early intervention can often lead to better outcomes, especially if there is an underlying condition that needs to be addressed.
