Post-Wisdom Tooth Extraction: Blackened Teeth and Tongue - Dentistry

After Wisdom Tooth Extraction

I had my upper left wisdom tooth extracted the day before yesterday.
The dentist did not stitch it up, and there is currently no swelling or pain.
However, the tooth in front of the extracted wisdom tooth has turned black, and the left side of my tongue has also turned black.
This situation started the day after the extraction and cannot be washed away.
What could be the issue? Thank you, Dr.
Chang!

Dear user, based on your description, you underwent a left upper wisdom tooth extraction, and postoperatively, the front teeth and the left side of your tongue have turned black.
This is likely due to a blood clot or bruising.
Regarding the discoloration of the front teeth, it is advisable to consult a dentist to check for potential cavities.
Additionally, you may want to have your tongue examined by a dentist to rule out any other abnormalities.
Thank you for your inquiry.

After having a wisdom tooth extracted, it's not uncommon to notice changes in the surrounding teeth and oral tissues. The blackening of the tooth adjacent to the extraction site and the darkening of the tongue can be concerning, but they can often be explained by a few common factors.

Firstly, the black coloration of the adjacent tooth could be due to several reasons. One possibility is that the tooth has been affected by dental caries (cavities) that were previously unnoticed. If the tooth had existing decay, the trauma from the extraction could have exacerbated the condition, making it more visible. Another possibility is that the tooth is experiencing a condition known as "tooth discoloration," which can occur due to various factors, including the presence of dental plaque, tartar, or even staining from food and beverages.
Additionally, the extraction process can sometimes lead to changes in the oral environment. For instance, if the extraction site is healing and there is a lack of proper oral hygiene, bacteria can accumulate, leading to discoloration. It's essential to maintain good oral hygiene practices, including gentle brushing and rinsing with salt water, to help keep the area clean and promote healing.

As for the darkening of the tongue, this can be attributed to a condition known as "black hairy tongue" (lingua villosa), which occurs when the filiform papillae on the tongue become elongated and discolored due to a buildup of dead cells, bacteria, and food particles. This condition can be exacerbated by factors such as poor oral hygiene, smoking, certain medications, or even dehydration. After a dental procedure, it’s common for patients to experience changes in their oral hygiene routine, which can contribute to this condition.

To address these issues, here are some recommendations:
1. Maintain Oral Hygiene: Continue to brush your teeth at least twice a day and floss daily. Pay special attention to the area around the extraction site, but be gentle to avoid disturbing the healing tissue.

2. Rinse with Salt Water: After the first 24 hours post-extraction, you can start rinsing your mouth with a warm saltwater solution (1 teaspoon of salt in 8 ounces of water) to help reduce inflammation and promote healing.

3. Stay Hydrated: Drink plenty of water to help keep your mouth moist and wash away food particles and bacteria.

4. Avoid Irritants: Refrain from smoking or consuming alcohol, as these can irritate the extraction site and contribute to discoloration.

5. Consult Your Dentist: If the discoloration persists or worsens, or if you experience any pain, swelling, or signs of infection (such as pus or fever), it’s crucial to return to your dentist for a thorough examination. They may need to assess the adjacent tooth for decay or other issues and provide appropriate treatment.

In summary, while the changes you are experiencing after your wisdom tooth extraction can be concerning, they are often manageable with proper care and attention. If you have any doubts or if the situation does not improve, do not hesitate to reach out to your dental professional for further evaluation and guidance.
