There is a feeling of bloating in the lower right abdomen, and there is a palpable mass upon palpation?
Hello Doctor: I am a 25-year-old female.
Recently, I have been experiencing a feeling of bloating in my lower right abdomen, specifically about three fingerbreadths above the right groin area.
When I press on this area, I can feel something that I am unsure about.
Could it be pressing on the ovary? I also experience a dull pain when I press on this area, which is more pronounced than in the left lower abdomen, and I feel a noticeable ache in my lower right abdomen while walking.
I have observed that my stool is shaped like a single strand, and it appears somewhat like Michelin Man stool, with some white liquid present in parts of the stool.
Doctor, could the bloating in my lower right abdomen be related to an ovarian issue, or could it be due to stool accumulation in the right lower abdomen? Or is there a problem with my intestines?

Hello! Thank you for reaching out with your concerns regarding the right lower abdominal bloating and discomfort you are experiencing. Given your age and the symptoms you've described, there are several potential causes for your discomfort that we can explore.
Firstly, the right lower abdomen is home to several important structures, including the appendix, parts of the intestines, and in females, the right ovary and fallopian tube. The sensation of bloating and the presence of a palpable mass could be indicative of various conditions.
1. Ovarian Issues: Since you mentioned the possibility of pressing on the ovary, it is important to consider ovarian cysts or other ovarian conditions. Ovarian cysts are fluid-filled sacs that can develop on the ovaries and may cause discomfort, especially if they become large or rupture. Symptoms can include bloating, pain during certain movements, and sometimes changes in bowel habits.
2. Gastrointestinal Concerns: The sensation of bloating could also be related to gastrointestinal issues. Conditions such as irritable bowel syndrome (IBS), constipation, or even diverticulitis can lead to bloating and discomfort in the lower abdomen. The fact that you mentioned your stool shape and consistency, along with the presence of white liquid, could suggest a dietary issue or a possible infection. It's essential to monitor your bowel habits and any changes in your stool.
3. Appendicitis: While less common, appendicitis can present with pain in the right lower abdomen, often accompanied by nausea, vomiting, and changes in appetite. If the pain becomes severe or is accompanied by fever, it would be crucial to seek medical attention immediately.
4. Hernia: Another possibility is an inguinal hernia, which can occur when tissue protrudes through a weak spot in the abdominal muscles. This can cause a noticeable bulge and discomfort, especially when straining or lifting.
5. Pelvic Inflammatory Disease (PID): If there is any possibility of a sexually transmitted infection or other pelvic infection, PID could cause pain and bloating in the lower abdomen. This condition often requires prompt treatment to prevent complications.
Given the complexity of your symptoms, it is crucial to consult with a healthcare provider for a thorough evaluation. They may recommend imaging studies, such as an ultrasound or CT scan, to assess the ovaries and other abdominal structures. Additionally, a physical examination can help determine the nature of the palpable mass you mentioned.
In the meantime, keeping a symptom diary can be helpful. Note when the pain occurs, any associated symptoms (like changes in bowel habits, nausea, or menstrual irregularities), and any dietary changes. This information can provide valuable insights for your healthcare provider.
Lastly, while anxiety can exacerbate physical symptoms, it's essential to focus on identifying any underlying medical issues. Addressing your physical health concerns can often alleviate anxiety related to health.
In summary, your symptoms could stem from various causes, including ovarian issues, gastrointestinal problems, or other abdominal conditions. A thorough evaluation by a healthcare professional is necessary to determine the exact cause and appropriate treatment. Please do not hesitate to seek medical attention, especially if your symptoms worsen or if you experience new symptoms such as fever, severe pain, or changes in bowel habits. Your health is important, and getting the right diagnosis is the first step toward feeling better.
