Erectile dysfunction issues?

Hello Doctor, I currently have an issue with erectile dysfunction that I would like to discuss with you.
Last September, I experienced tinnitus and hearing loss, and I have been receiving treatment from both traditional Chinese medicine and Western medicine until early February of this year.
Before early February, I did not have any issues with erectile dysfunction.
However, recently in February, I went to a clinic in Taipei for intravenous drip therapy, which included B vitamins, vasodilators, and glucose.
After the treatment, I immediately felt the urge to urinate (similar to the effects of a diuretic), and I received the injections twice a week.
After the third injection, I noticed that I was unable to achieve an erection during intercourse, so I immediately stopped the injections.

In the mornings, I sometimes wake up with an erection, and I have tried to have intercourse at that time, but it becomes soft after a short while.
When I try to achieve an erection on my own, it takes a long time, and even after I do, it becomes soft shortly after.
Am I experiencing erectile dysfunction? Additionally, I occasionally have sharp pains in my lower back near the kidneys.
Could this issue be caused by the intravenous drips? Is there a possibility that my kidney function is also compromised? This problem has indirectly led to feelings of depression, anxiety, and insomnia, which is very distressing for me.
I would appreciate your insights on this matter.

Based on the situation, it is indeed possible that there is erectile dysfunction, which may not necessarily be related to medication.
However, depression, anxiety, insomnia, and excessive stress can all negatively impact erectile function.

Erectile dysfunction (ED) is a common condition that can affect men of all ages, but it is particularly prevalent among older men. Understanding the causes and treatment options for ED is crucial for managing this condition effectively. Based on your description, it seems you are experiencing a range of symptoms that may be related to both physical and psychological factors.

Causes of Erectile Dysfunction
1. Physical Causes:
- Vascular Issues: Conditions that affect blood flow, such as hypertension, diabetes, and high cholesterol, can lead to ED. The fact that you have undergone treatments involving vasodilators suggests that blood flow may be a concern.

- Hormonal Imbalances: Low testosterone levels can contribute to ED. Hormonal evaluations may be necessary to rule out this cause.

- Neurological Conditions: Nerve damage from conditions like diabetes or even from certain medical treatments can impair erectile function.

- Medication Side Effects: Some medications, including those for hypertension or depression, can lead to ED. The injections you received may have had side effects that contributed to your symptoms.

2. Psychological Causes:
- Anxiety and Depression: Psychological factors can significantly impact sexual performance. Your mention of feeling depressed and anxious about your condition is important, as these feelings can create a cycle that exacerbates ED.

- Stress: Stress from various life situations can also contribute to ED.

3. Lifestyle Factors:
- Substance Use: Alcohol and drug use can impair sexual function.

- Physical Health: Obesity, lack of exercise, and poor diet can also contribute to ED.

Treatment Options
1. Lifestyle Modifications:
- Diet and Exercise: A healthy diet and regular physical activity can improve overall health and blood flow, which may help alleviate ED.

- Weight Management: If overweight, losing weight can improve erectile function.

2. Medical Treatments:
- Oral Medications: Phosphodiesterase type 5 inhibitors (such as Viagra, Cialis) are commonly prescribed for ED and can be effective.

- Hormonal Therapy: If low testosterone is identified, hormone replacement therapy may be an option.

- Injection Therapy: While you have experienced issues with the injections, they are sometimes effective for ED. It’s essential to discuss any side effects with your doctor.

3. Psychological Counseling:
- If anxiety or depression is contributing to your ED, therapy or counseling may be beneficial. Cognitive-behavioral therapy (CBT) can help address these issues.

4. Alternative Treatments:
- Some men find success with vacuum erection devices or penile implants, but these are typically considered when other treatments have failed.

Follow-Up and Monitoring
Given your recent experiences with injections and the onset of ED, it is crucial to follow up with your healthcare provider. They can evaluate your kidney function, especially since you mentioned experiencing pain in the kidney area. This pain could be unrelated to your ED but should be assessed to rule out any underlying issues.

Conclusion
Erectile dysfunction can be a complex issue with multiple contributing factors. It is essential to approach this condition holistically, considering both physical and psychological aspects. Open communication with your healthcare provider is key to finding the most effective treatment plan for your specific situation. If you are experiencing persistent symptoms, do not hesitate to seek further evaluation and support. Remember, you are not alone in this, and many effective treatments are available.
