Testicular pain..?
Hello Doctor: Recently, I have been experiencing some swelling and pain in my right testicle occasionally, but it returns to normal after a few days.
However, it sometimes starts to swell and hurt again, and it is always on the right side.
Could this be due to wearing tight pants, or is there another issue? Could you please help me with this? Thank you!

Dr. Liu Jianting reply Urology
Is there any swelling? Do you have a fever? Does it still hurt if your pants are a bit loose? Please don't neglect your condition; consult a urologist for a thorough examination.

More Info
Right testicular pain can be concerning and may arise from various causes. It’s essential to understand that while tight clothing can contribute to discomfort, there are several other potential underlying issues that could lead to the symptoms you are experiencing.
Common Causes of Right Testicular Pain
1. Epididymitis: This is an inflammation of the epididymis, the tube that stores and carries sperm. It can be caused by infections, including sexually transmitted infections (STIs) or urinary tract infections (UTIs). Symptoms may include swelling, tenderness, and pain in the affected testicle.
2. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off the blood supply to the testicle. Symptoms typically include sudden, severe pain and swelling. If you experience this, seek immediate medical attention.
3. Inguinal Hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. It can cause discomfort or pain in the groin and may radiate to the testicle.
4. Varicocele: This is an enlargement of the veins within the scrotum, similar to varicose veins. It can cause a dull ache or a feeling of heaviness in the affected testicle, often worsening with prolonged standing or physical activity.
5. Hydrocele: This is a fluid-filled sac surrounding a testicle, leading to swelling in the scrotum. While usually painless, it can cause discomfort if it becomes large.
6. Trauma or Injury: Any direct injury to the testicles can result in pain and swelling. This could be from sports, accidents, or other physical activities.
7. Testicular Cancer: Although less common, persistent pain or discomfort in the testicle can be a sign of testicular cancer. It’s essential to monitor for any lumps or changes in size.
Possible Connection to Tight Clothing
Wearing tight clothing can indeed contribute to discomfort in the testicular area. Tight pants or underwear can restrict blood flow and cause irritation, leading to a sensation of pain or heaviness. However, if the pain persists or is accompanied by other symptoms such as swelling, redness, or fever, it is crucial to consult a healthcare professional for a thorough evaluation.
Recommendations
1. Monitor Symptoms: Keep track of when the pain occurs, its intensity, and any associated symptoms. This information can be helpful for your healthcare provider.
2. Wear Loose Clothing: Consider wearing looser-fitting underwear and pants to see if this alleviates your symptoms.
3. Seek Medical Evaluation: If the pain persists, worsens, or is accompanied by other symptoms (such as swelling, fever, or changes in urination), it is essential to see a healthcare provider. They may perform a physical examination and possibly order imaging studies or lab tests to determine the cause of your pain.
4. Avoid Self-Diagnosis: While it’s helpful to gather information, avoid jumping to conclusions about the cause of your pain without professional input.
5. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort, but consult with a healthcare provider before starting any medication, especially if you have underlying health conditions or are taking other medications.
In summary, while tight clothing may contribute to discomfort, various medical conditions could be responsible for your right testicular pain. It’s essential to monitor your symptoms and consult a healthcare professional for a proper diagnosis and treatment plan.
