Sudden dizziness?

Yesterday, while walking through the night market, I felt like my ears were blocked, and my vision became increasingly dark until I couldn't see anything.
My arms and legs also felt weak.
This isn't the first time; it has happened two or three times before.
What symptoms could this be? How can I address this?

Lin Pipi, 10~19 year old female. Ask Date: 2016/03/06

Dr. Jiang Junyi reply Neurology

Based on your description: The symptoms you mentioned of sudden weakness are more likely caused by syncope, which may result from a drop in blood pressure or vasoconstriction leading to reduced cerebral blood flow and subsequent cerebral ischemia.
Syncope is more likely to occur when a person is standing, as this position results in the least cerebral blood flow and a higher cardiac output.
However, when there is a decrease in cerebral blood flow, the body may not be able to respond quickly enough, leading to syncope.
The occurrence is less frequent when sitting and almost nonexistent when lying down.
It is recommended that you discuss examination and treatment options with a neurologist.
Wishing you good health and peace.

Sudden dizziness, especially when accompanied by symptoms such as visual disturbances, weakness in the limbs, and a sensation of ear fullness, can be concerning and may indicate several underlying medical conditions. The symptoms you described, particularly the feeling of your ears being blocked and the visual changes leading to temporary blindness, suggest that you may be experiencing a form of vertigo or a vestibular disorder.
One possible cause of your symptoms could be benign paroxysmal positional vertigo (BPPV), which occurs when tiny calcium crystals in the inner ear become dislodged and disrupt the normal balance signals sent to the brain. This condition often leads to brief episodes of dizziness, especially when changing positions. Another potential cause could be vestibular neuritis, an inflammation of the vestibular nerve, which can cause sudden, severe dizziness and balance issues.

Additionally, the sensation of fullness in the ears could indicate Eustachian tube dysfunction, where the tube that connects the middle ear to the back of the throat becomes blocked, leading to pressure changes and dizziness. This can happen due to allergies, sinus infections, or even changes in altitude.

Given that you have experienced these symptoms multiple times, it is essential to seek medical attention. A healthcare provider can perform a thorough evaluation, including a physical examination and possibly imaging studies or balance tests, to determine the underlying cause of your dizziness. They may also assess for other conditions such as migraines, which can sometimes present with dizziness and visual disturbances, or cardiovascular issues that could lead to transient ischemic attacks (TIAs), which are often referred to as "mini-strokes."
In terms of immediate solutions, if you experience dizziness, it is advisable to sit or lie down until the sensation passes. Avoid sudden movements and try to stay hydrated. If you notice that certain positions trigger your dizziness, it may be helpful to avoid those movements until you can consult a healthcare professional.

Preventive measures can include lifestyle modifications such as staying well-hydrated, managing stress, and avoiding triggers that may lead to dizziness, such as rapid head movements or certain visual stimuli. If your dizziness is related to an underlying condition, your healthcare provider may recommend specific treatments or therapies tailored to your diagnosis.

In summary, while occasional dizziness can be benign, recurrent episodes warrant further investigation to rule out serious conditions. It is crucial to consult with a healthcare professional who can provide a comprehensive assessment and appropriate management based on your specific symptoms and medical history.
