Eye problems
Hello, doctor.
My husband has myopia (350 degrees and 450 degrees) and originally had convergence insufficiency.
Recently, he was diagnosed with a slight exotropia.
He has tried using prisms, but it was unsuccessful.
He has been experiencing visual fatigue for six years, and his fundus examination is currently normal.
Whenever he wears glasses, he feels uncomfortable and experiences overall discomfort.
His eye fatigue is becoming increasingly difficult to alleviate.
Not wearing glasses makes him tired, but wearing them makes him even more fatigued.
A year and a half ago, he would get extremely tired after looking at the computer for just five minutes, and now even a glance at the computer exhausts him.
What could be the underlying issue? Is it related to the nervous system or has his eyes become fatigued? How can he alleviate this? Additionally, he experiences significant afterimages; when he looks at bright objects, they linger in his vision for a long time.
There has been a slight decline in his vision.
Thank you.

Hello, based on your description, it seems that issues such as glasses, eye fatigue, and dry eyes are all possible.
It is recommended to use your eyes appropriately and allow them to rest.
If the symptoms persist, you should see an ophthalmologist for an examination.

Understanding eye strain and the various vision problems associated with it is crucial for maintaining eye health, especially in today's digital age where prolonged screen time is common. Your husband's situation, characterized by myopia (nearsightedness), insufficient convergence, and recently diagnosed exotropia (outward strabismus), presents a complex interplay of visual challenges that can lead to significant discomfort and strain.
Causes of Eye Strain:
1. Refractive Errors: Myopia is a common refractive error where distant objects appear blurry. This condition can lead to eye strain as the eyes work harder to focus. Insufficient convergence, which is the ability of the eyes to work together to focus on a near object, can exacerbate this strain, especially during tasks like reading or using a computer.
2. Strabismus: The recent diagnosis of exotropia indicates that the eyes are not properly aligned. This misalignment can cause double vision and difficulty in focusing, leading to increased fatigue as the brain struggles to reconcile the different images from each eye.
3. Digital Eye Strain: Prolonged use of computers and digital devices can lead to symptoms of digital eye strain, including discomfort, dryness, and blurred vision. The symptoms can worsen if the individual has underlying conditions like myopia or strabismus.
4. Visual Fatigue: Over time, the muscles that control eye movement can become fatigued from constant use, especially if there are existing issues like insufficient convergence or strabismus. This fatigue can manifest as discomfort, headaches, and difficulty focusing.
5. Neurological Factors: While the primary issues seem to be ocular, there can be neurological components involved, especially if there is persistent discomfort and visual disturbances like afterimages (the phenomenon where a bright object leaves a lingering image in the visual field).
Solutions and Recommendations:
1. Comprehensive Eye Examination: It is essential for your husband to undergo a thorough eye examination by an ophthalmologist or optometrist. This should include assessments for refractive errors, eye alignment, and overall eye health. The eye care professional may recommend corrective lenses tailored to his specific needs, including prism glasses if necessary.
2. Vision Therapy: Given the diagnosis of exotropia and convergence insufficiency, vision therapy may be beneficial. This therapy involves exercises designed to improve eye coordination and focusing abilities, potentially alleviating some of the strain and discomfort.
3. Ergonomic Adjustments: Encourage your husband to make ergonomic adjustments to his workspace. This includes ensuring that the computer screen is at eye level, maintaining an appropriate distance from the screen, and using proper lighting to reduce glare.
4. The 20-20-20 Rule: To combat digital eye strain, he should follow the 20-20-20 rule: every 20 minutes, take a 20-second break and look at something 20 feet away. This helps relax the eye muscles and reduce fatigue.
5. Regular Breaks and Eye Exercises: Encourage him to take regular breaks from screens and engage in eye exercises that promote relaxation and flexibility of the eye muscles. Simple exercises include rolling the eyes, focusing on near and far objects, and blinking frequently to keep the eyes moist.
6. Hydration and Humidification: Ensure he stays hydrated and consider using a humidifier in his workspace to combat dry eyes, which can exacerbate discomfort.
7. Consultation with Specialists: If symptoms persist despite these interventions, it may be beneficial to consult with a neurologist to rule out any underlying neurological issues contributing to the visual disturbances.
In conclusion, addressing eye strain and associated vision problems requires a multifaceted approach that includes proper diagnosis, corrective measures, ergonomic adjustments, and regular eye care. By taking proactive steps, your husband can improve his visual comfort and overall quality of life.
