Bloating accompanied by pain in the lower right back?

Hello Doctor: In February of this year, a CT scan revealed a urinary stone in the left ureter.
A few weeks later, I passed it on my own, and a subsequent hospital examination showed no hydronephrosis in the left kidney.
Recently, in March, I have been frequently dining out and drinking alcohol, and I often experience bloating in my abdomen, followed by persistent burping and a dull ache in the right lower back.
This sensation is similar to the pain I felt previously with the left kidney stone, but it resolves the next day.
I would like to know if abdominal bloating can cause right lower back pain.
Is the right lower back pain more likely related to a muscular issue or a stone issue?

Mr. Li, 30~39 year old female. Ask Date: 2016/03/07

Dr. Li Xinyi reply Urology

Patients with kidney stones should promptly return to the urology department for evaluation whenever they feel any unusual symptoms.

Bloating can indeed cause discomfort in various areas of the body, including the lower back. The connection between bloating and right-sided lower back pain can be attributed to several factors, including muscle tension, referred pain, and gastrointestinal issues.

When you experience bloating, your abdomen becomes distended due to the accumulation of gas or fluid. This distension can put pressure on the surrounding structures, including the diaphragm and the muscles of the lower back. The increased pressure in the abdominal cavity can lead to muscle tension as your body tries to compensate for the discomfort, which may result in pain or discomfort in the lower back, particularly on the right side if that is where the pressure is most pronounced.

Additionally, the nerves that supply the abdominal organs also supply the lower back. This means that irritation or inflammation in the gastrointestinal tract can lead to referred pain in the lower back. For instance, if your bloating is due to digestive issues, such as gas buildup or constipation, the discomfort can manifest as pain in the lower back.

In your case, given your history of kidney stones, it’s essential to consider whether the pain you are experiencing is related to your previous condition. While it is possible that the bloating is causing muscle tension and discomfort, it is also important to rule out any potential issues related to the urinary tract. Kidney stones can sometimes cause intermittent pain that may be mistaken for muscular pain, especially if the pain is not constant and varies in intensity.

Since you mentioned that the pain is similar to what you experienced with your previous kidney stone issue, it would be prudent to monitor your symptoms closely. If the pain persists, worsens, or is accompanied by other symptoms such as changes in urination, fever, or severe discomfort, it would be advisable to seek medical attention. A healthcare professional may recommend imaging studies or further evaluation to rule out any complications related to your previous kidney stones.

In the meantime, managing your bloating may help alleviate some of the discomfort in your lower back. Here are some strategies you might consider:
1. Dietary Adjustments: Pay attention to foods that may cause bloating, such as carbonated beverages, high-fiber foods, and certain carbohydrates. Keeping a food diary can help identify triggers.

2. Hydration: Ensure you are drinking enough water, as dehydration can lead to constipation, which may exacerbate bloating.

3. Physical Activity: Gentle exercise, such as walking or stretching, can help promote digestion and reduce bloating.

4. Posture: Maintaining good posture can help alleviate pressure on your back and abdomen.

5. Heat Therapy: Applying a warm compress to your abdomen may help relieve bloating and associated discomfort.

6. Over-the-Counter Remedies: Antacids or simethicone-based products may help relieve gas and bloating.

7. Relaxation Techniques: Stress can exacerbate gastrointestinal issues, so practices such as deep breathing, yoga, or meditation may be beneficial.

In conclusion, while bloating can contribute to right-sided lower back pain, it is essential to consider your medical history and monitor your symptoms closely. If you have any concerns or if the pain persists, consulting with a healthcare professional is the best course of action to ensure proper diagnosis and treatment.
