My finger has become stiff; will surgery help?

I had a car accident on November 29th last year, and my middle finger, ring finger, and little finger have become stiff and cannot bend.
The bones have shifted into the palm of my hand.
It has been over three months now.
Will surgery help? After the surgery, will my fingers regain mobility? Thank you.

Wu, 50~59 year old female. Ask Date: 2016/03/07

Dr. Shi Guozheng reply Orthopedics

It may be necessary to have a physician evaluate the situation, as surgery may not necessarily restore the joint to its original level of flexibility.

After an accident, particularly one that results in stiffness and immobility of the fingers, the question of whether surgery can restore flexibility is complex and depends on several factors. In your case, you mentioned that your middle, ring, and little fingers have become stiff and cannot bend, and that there is a concern about bone displacement into the palm. Given that it has been over three months since the accident, it is crucial to assess the extent of the injury and the current state of your fingers.

Surgery may be necessary if there are structural issues, such as bone fragments impinging on the tendons or joints, or if there is significant scarring or adhesions that prevent movement. Surgical intervention can involve several procedures, including realignment of bones, removal of bone fragments, or even tendon repair if they have been damaged. The goal of surgery would be to restore the anatomical integrity of the fingers and allow for better mobility.

However, it is important to note that surgery does not guarantee a full return to pre-injury flexibility. The success of the surgery and the restoration of function depend on various factors, including:
1. Extent of Injury: The more severe the injury, the more complex the surgical procedure may need to be. If there is significant damage to the tendons, ligaments, or joints, recovery may be more challenging.

2. Timing of Surgery: Ideally, surgery should be performed as soon as possible after an injury to minimize complications such as stiffness and loss of range of motion. Delayed surgery can lead to more extensive scarring and stiffness.

3. Rehabilitation: Post-surgical rehabilitation is critical. Engaging in physical therapy and following a structured rehabilitation program can significantly enhance recovery outcomes. This may include exercises to improve range of motion, strength training, and techniques to reduce scar tissue formation.

4. Individual Healing Response: Each person's body heals differently. Factors such as age, overall health, and adherence to rehabilitation protocols can influence recovery.

5. Surgical Technique: The skill and experience of the surgeon play a vital role in the outcome. A surgeon specialized in hand injuries will have a better understanding of the intricacies involved in restoring function to the fingers.

In conclusion, while surgery may provide an opportunity to restore some degree of flexibility to your fingers, it is essential to have a thorough evaluation by an orthopedic or hand surgeon. They will assess your specific situation, including imaging studies like X-rays or CT scans, to determine the best course of action. Post-surgery, a dedicated rehabilitation program will be crucial for maximizing your recovery and regaining as much function as possible. Always discuss your concerns and expectations with your healthcare provider to make informed decisions about your treatment options.
