A lump has appeared near the hair-bearing area of the anus?

Recently, I noticed a small bump about 0.1 cm in size near the hair follicles around my anus.
It resembles a pimple but is not painful and has no other symptoms.
My bowel movements are normal without pain or itching.
I previously had an anal fistula, but it was surgically removed by a doctor using ultrasound, and it has been six months since the procedure.
I would like to know what this bump could be.

Dr. Ke Fangxu reply Surgery

Perianal masses require a physical examination for diagnosis; however, most are benign lesions that do not warrant concern.
If there is inflammation, oral medication may be necessary.
If symptoms do not improve or cause disruption to daily activities, it is important to consult a physician for an accurate diagnosis.

When you notice a small bump near the anus, especially one that resembles a pimple and is painless with no accompanying symptoms, it can be concerning. However, based on your description, there are several potential explanations for this bump.

1. Sebaceous Cyst: One common possibility is a sebaceous cyst, which is a small, non-cancerous lump beneath the skin. These cysts are filled with a fatty substance and can occur anywhere on the body, including near the anus. They are usually painless and can sometimes resolve on their own.

2. Skin Tag: Another possibility is a skin tag, which is a small, benign growth of skin that can appear in various areas, including the anal region. Skin tags are typically harmless and may not require treatment unless they become bothersome.

3. Pilonidal Cyst: Given your history of anal fistula, a pilonidal cyst could also be a consideration, especially if it is located near the midline of the buttocks. These cysts can develop in the crease of the buttocks and may become infected, but they can also remain asymptomatic.

4. Hemorrhoids: While hemorrhoids typically present with pain, itching, or bleeding, sometimes they can appear as small lumps around the anal area. If there are no symptoms, it is less likely to be hemorrhoids, but it is still a possibility.

5. Anal Warts: Human papillomavirus (HPV) can cause warts in the anal area. These may appear as small bumps and can be painless. If you have had any risk factors for HPV, this could be a consideration.

6. Recurrence of Fistula or Abscess: Since you have a history of anal fistula, it is essential to consider the possibility of a recurrence or the formation of an abscess. If the bump changes in size, becomes painful, or if you notice any discharge, it would be prudent to seek medical attention.

Given that the bump is small (0.1 cm) and asymptomatic, it may not require immediate medical intervention. However, it is essential to monitor it for any changes in size, color, or symptoms. If you notice any discomfort, bleeding, or if the bump increases in size, it would be wise to consult a healthcare professional for a thorough evaluation.

In summary, while the bump you described is likely benign, it is always best to err on the side of caution, especially with a history of anal conditions. A healthcare provider can perform a physical examination and, if necessary, imaging or biopsy to determine the exact nature of the bump. Regular follow-ups and being aware of any changes in your body are crucial for maintaining your health.
