Penile glans enlargement?
Recently, I have noticed swelling of the glans penis, along with some "broken skin" on the inner side of the foreskin and the glans itself.
It is painful when touched.
What could be the cause of this?

Dr. Song Zhixian reply Urology
The glans and foreskin have good blood circulation.
Generally, if there is trauma that breaks the skin, it heals quickly.
However, if the foreskin is too long and completely covers the glans, it can lead to poor ventilation and moisture retention.
The presence of sloughed epithelial tissue and secretory glands can promote bacterial and fungal growth if not cleaned regularly, leading to balanitis and posthitis.
This may present with skin breakdown and red lesions.
Topical antibiotics and antifungal creams can be used.
If there is inappropriate sexual contact, especially with ulcer formation, testing for sexually transmitted infections is necessary for further treatment.

Swelling and pain in the glans (the head of the penis) can be concerning symptoms and may arise from various causes. Understanding these causes is essential for determining the appropriate treatment and management. Here are some potential reasons for the symptoms you are experiencing:
1. Infections: One of the most common causes of swelling and pain in the glans is an infection. This could be a sexually transmitted infection (STI) such as herpes, gonorrhea, or chlamydia. Bacterial infections can also lead to conditions like balanitis, which is inflammation of the glans, often caused by poor hygiene, irritants, or infections.
2. Balanitis: This condition specifically refers to inflammation of the glans and can be caused by various factors, including yeast infections (Candida), bacterial infections, or irritants such as soaps, lotions, or condoms. Symptoms typically include redness, swelling, and pain, and sometimes a discharge.
3. Allergic Reactions: Allergies to certain products, such as soaps, lubricants, or latex in condoms, can cause irritation and swelling of the glans. If you have recently changed any products you use, this could be a potential cause.
4. Skin Conditions: Conditions such as psoriasis or eczema can affect the genital area, leading to symptoms like swelling, redness, and pain. These conditions may also cause skin breakdown, leading to the "broken skin" appearance you described.
5. Trauma or Friction: Physical trauma or excessive friction during sexual activity or masturbation can lead to swelling and pain. If you have engaged in activities that may have caused trauma, this could be a contributing factor.
6. Phimosis or Paraphimosis: If the foreskin cannot be retracted fully over the glans (phimosis), it can lead to swelling and pain. Paraphimosis, where the retracted foreskin cannot return to its normal position, can also cause significant swelling and pain.
Solutions and Recommendations:
1. Seek Medical Attention: Given the symptoms you are experiencing, it is crucial to consult a healthcare provider, preferably a urologist. They can perform a physical examination, take a detailed medical history, and possibly conduct tests to determine the underlying cause of your symptoms.
2. Avoid Irritants: Until you can see a doctor, avoid using any new soaps, lotions, or other products that may irritate the area. Stick to gentle, unscented products.
3. Maintain Hygiene: Keep the area clean and dry. Gently wash the glans and foreskin with warm water and mild soap, and ensure you dry the area thoroughly afterward.
4. Avoid Sexual Activity: If you are experiencing pain, it may be wise to refrain from sexual activity until you have been evaluated by a healthcare professional.
5. Topical Treatments: Depending on the diagnosis, your doctor may recommend topical antifungal or antibacterial creams if an infection is suspected. For allergic reactions, a mild corticosteroid cream may be prescribed to reduce inflammation.
6. Follow-Up: If you are prescribed treatment, ensure you follow up with your healthcare provider to monitor your symptoms and adjust treatment as necessary.
In summary, swelling and pain in the glans can result from various causes, including infections, skin conditions, or trauma. It is essential to seek medical evaluation to determine the exact cause and receive appropriate treatment. Early intervention can help prevent complications and promote healing.
