Coughing at night for more than a month?
Hello, doctor! I have been experiencing a cough for over a month now, which usually occurs at night while I am sleeping, especially when lying flat.
I hardly cough during the day, and I do not have any chest pain.
The color of the phlegm is white, and some of it is sticky (it doesn't wash away easily).
I also have symptoms of nasal allergies (I wake up with nasal congestion every morning).
I am wondering if the cough could be caused by post-nasal drip? Additionally, I was previously diagnosed with Mycoplasma pneumonia; could this be related to the Mycoplasma infection not being fully resolved? Thank you, doctor!

Dear Zhih-Lun: Based on the symptoms you described, it seems more like an allergic cough.
You might want to wash all your bedding and pillows, and avoid having clutter around your bed.
After these measures, if there is still no improvement, you can come to the hospital for allergy testing.
Changhua Hospital cares about you.

Nighttime cough that persists for over a month can be concerning, especially when it primarily occurs while lying down and is accompanied by symptoms such as nasal congestion and white, sticky phlegm. Based on your description, there are several potential causes to consider, including post-nasal drip, allergic rhinitis, and possibly residual effects from a previous Mycoplasma pneumonia infection.
Post-Nasal Drip: Given your history of nasal allergies and the fact that you experience nasal congestion in the mornings, post-nasal drip is a likely culprit for your nighttime cough. When you lie down, mucus can accumulate in the back of your throat, leading to irritation and coughing. This is often exacerbated by allergens in your environment, such as dust mites in bedding or pillows. It would be beneficial to ensure that your sleeping environment is clean and free of allergens. Regularly washing your bedding in hot water, using hypoallergenic pillow covers, and maintaining low humidity levels can help reduce exposure to allergens.
Allergic Rhinitis: Your symptoms of nasal congestion and the timing of your cough suggest that allergic rhinitis could be contributing to your nighttime discomfort. Allergies can lead to inflammation of the nasal passages, which can cause mucus production and subsequent coughing. Over-the-counter antihistamines or nasal corticosteroids may provide relief from these symptoms. Consulting with an allergist for testing and tailored treatment options could also be beneficial.
Mycoplasma Pneumonia: While Mycoplasma pneumonia can cause respiratory symptoms, including cough, it is less likely to be the sole cause of your persistent nighttime cough if you have already been diagnosed and treated. However, if your symptoms have not fully resolved, it may be worth discussing with your healthcare provider whether further evaluation or treatment is necessary. Chronic cough can sometimes linger after an infection has resolved, especially if there was significant airway inflammation.
Other Considerations: Other potential causes of nighttime cough include gastroesophageal reflux disease (GERD), which can cause acid to irritate the throat when lying down, and chronic bronchitis, particularly if you have a history of smoking or exposure to irritants. If your cough is accompanied by any new symptoms, such as fever, weight loss, or changes in sputum color, it is essential to seek medical attention promptly.
In summary, your nighttime cough could be attributed to post-nasal drip related to allergies, and it may not be directly linked to Mycoplasma pneumonia if you have already been treated. It is advisable to consult with a healthcare provider for a thorough evaluation, including a physical examination and possibly allergy testing, to determine the underlying cause of your symptoms and to develop an appropriate treatment plan. Managing your allergies and ensuring a clean sleeping environment may significantly improve your nighttime symptoms.
