It's frustrating to feel unwell without being able to identify the cause?

Hello doctor, for the past couple of years, the issues with my glans and foreskin have caused me significant distress.
Initially, I sought medical attention due to itching and a stinging sensation, and the doctor suggested it might be eczema and prescribed a topical medication.
However, my condition did not improve; instead, it became red and swollen, and I even experienced ulceration on the surface of the glans.
I underwent blood tests, urine tests, tests for sexually transmitted infections, and diabetes screening, all of which returned normal results.
Despite this, the effectiveness of the medications has been limited, and the doctor only provided me with ointments, advising me to return if I had further issues.
Does this mean I have to continue applying medication indefinitely? Recently, I have been using a medication called Meikang, which was initially tolerable, but later I began to experience stinging and itching again, with the surface of the glans becoming red and small red spots appearing.
These spots make me very anxious; sometimes they resolve after applying medication for a while, but other times they gradually enlarge and ulcerate, significantly affecting my mood.
What can I do to improve my condition?

Based on the symptoms, it is possible that this is recurrent balanitis.
It is recommended to improve lifestyle habits, avoid stress, refrain from smoking and alcohol, and avoid spicy and irritating foods.
Additionally, not staying up late can help reduce the frequency of recurrence.

Hello,
I understand that you're experiencing significant distress due to persistent urological discomfort, particularly involving your glans and foreskin. It's important to address both the physical symptoms and the emotional toll they are taking on you. Let's break down your situation and explore potential avenues for relief.

From your description, it seems that you've been dealing with symptoms such as itching, stinging, redness, and even ulceration of the glans. You've already consulted with healthcare professionals who have ruled out common conditions through blood tests, urinalysis, and screenings for sexually transmitted infections. However, the lack of improvement despite treatment can be frustrating and disheartening.

The initial diagnosis of eczema (or dermatitis) is a possibility, as skin conditions can manifest in various ways, including the genital area. However, the persistence and worsening of your symptoms suggest that there may be other underlying factors at play. Here are some considerations and recommendations:
1. Seek a Specialist: If you haven't already, consider consulting a urologist or a dermatologist who specializes in genital skin conditions. They may have more experience with complex cases and can offer a fresh perspective on your symptoms.

2. Consider Allergies or Irritants: Sometimes, skin reactions can be triggered by irritants or allergens. This could include soaps, detergents, or even certain fabrics. Keeping a diary of your symptoms in relation to your activities, hygiene products, and any new items you introduce can help identify potential triggers.

3. Topical Treatments: While you mentioned using a topical medication, it’s crucial to ensure that the treatment is appropriate for your specific condition. Corticosteroids may be prescribed for inflammation, but long-term use can lead to skin thinning. Your doctor may suggest alternative treatments, such as calcineurin inhibitors (like tacrolimus or pimecrolimus), which can be effective for inflammatory skin conditions without the side effects of steroids.

4. Hygiene Practices: Maintaining proper hygiene is essential, but be cautious not to over-cleanse the area, as this can exacerbate irritation. Use mild, unscented soaps and avoid harsh chemicals. After washing, ensure the area is thoroughly dried, as moisture can contribute to fungal infections.

5. Emotional Support: Chronic discomfort can lead to anxiety and emotional distress. It may be beneficial to speak with a mental health professional who can help you cope with the psychological impact of your symptoms. Techniques such as cognitive-behavioral therapy (CBT) can be effective in managing anxiety related to health concerns.

6. Follow-Up: Since your symptoms have not improved, it’s essential to follow up with your healthcare provider. They may consider a biopsy or other diagnostic tests to rule out conditions such as lichen sclerosus or other dermatological issues that could be affecting the genital area.

7. Lifestyle Modifications: Consider lifestyle changes that may help alleviate symptoms. This includes wearing loose-fitting, breathable underwear, avoiding irritants, and managing stress through relaxation techniques or mindfulness practices.

In conclusion, while it can be frustrating to deal with unexplained urological discomfort, there are steps you can take to seek relief. Consulting with specialists, exploring alternative treatments, and addressing both physical and emotional aspects of your condition can lead to better outcomes. Remember, you are not alone in this, and there are healthcare professionals who can help guide you toward a solution.

Take care, and I wish you the best in your journey toward relief and recovery.
