Irregular bowel movements

Doctor: I would like to ask about my bowel movements.
I used to have them every two to three days, and they were normal and formed.
However, in the past few months, they have become irregular, with a pellet-like consistency and varying sizes.
I have been drinking water and eating fruits and vegetables, trying various methods to aid in bowel movements, but the shape remains irregular.
Sometimes, I notice a reddish tint in the toilet water, and today there was also some red mucus, but I don’t experience any pain.
It doesn’t seem like constipation.
Which specialist should I see? Would a stool test be sufficient to determine the issue, or do I need to undergo a colonoscopy? I often feel like my bowels are not completely empty, and I sometimes have to use my hands to help facilitate a bowel movement.

Pei Pei, 20~29 year old female. Ask Date: 2016/03/13

Dr. Chen Qifeng reply Gastroenterology and Hepatology

When it comes to irregular bowel movements, it is essential to understand the potential underlying causes and when to seek medical help. Based on your description, it seems you have experienced a significant change in your bowel habits over the past few months, which is a crucial indicator that warrants further evaluation.

Firstly, the fact that your stools have changed in consistency and shape—from being normal, cylindrical stools to irregular, pellet-like shapes—can indicate several issues. These changes can be associated with dietary factors, dehydration, or even gastrointestinal disorders. The presence of red coloration in your stool, especially if it is accompanied by mucus, raises additional concerns. While you mentioned that you are not experiencing pain, the combination of these symptoms suggests that a thorough investigation is necessary.

When to Seek Medical Help
1. Persistent Changes: If you notice a consistent change in your bowel habits lasting more than a few weeks, it is advisable to consult a healthcare provider. Changes in stool shape, frequency, and consistency can be signs of underlying conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or even colorectal issues.

2. Blood in Stool: The presence of red coloration in your stool, especially if it appears alongside mucus, is a significant symptom that should not be ignored. Blood in the stool can indicate various conditions, ranging from hemorrhoids to more serious issues like polyps or colorectal cancer.

3. Feeling of Incomplete Evacuation: The sensation of not fully emptying your bowels can be distressing and may indicate a functional issue within the gastrointestinal tract. This symptom, combined with irregular stool patterns, should prompt a medical evaluation.

Recommended Steps
1. Consult a Gastroenterologist: Given your symptoms, it would be best to see a gastroenterologist. They specialize in digestive system disorders and can provide a comprehensive evaluation of your condition.

2. Diagnostic Tests: Your doctor may recommend several tests, including:
- Stool Tests: These can help identify infections, blood, or other abnormalities in your stool.

- Colonoscopy: If there are concerns about the presence of polyps, inflammation, or other structural issues in the colon, a colonoscopy may be necessary. This procedure allows the doctor to visualize the interior of your colon and take biopsies if needed.

3. Dietary Review: While you mentioned that you consume fruits and vegetables, it may be beneficial to review your overall diet with a nutritionist. Sometimes, specific dietary adjustments can significantly improve bowel regularity.

4. Hydration: Ensure you are drinking enough water daily, as dehydration can lead to harder stools and irregular bowel movements.

5. Monitor Symptoms: Keep a diary of your bowel habits, including the frequency, consistency, and any accompanying symptoms. This information can be invaluable for your healthcare provider in diagnosing your condition.

Conclusion
In summary, the changes in your bowel movements, particularly the irregular shapes and the presence of red coloration, warrant a medical evaluation. It is crucial to consult a gastroenterologist who can guide you through the necessary diagnostic steps and provide appropriate treatment options. Early intervention can lead to better outcomes, especially if there are underlying conditions that need to be addressed. Remember, your health is paramount, and seeking help is a proactive step towards maintaining your well-being.
