Postoperative stabbing pain after varicocele surgery?

Hello, Doctor Hsu.
I had left-sided varicocele surgery 12 days ago, and the incision is about 4 cm in the groin area.
After the surgery, the wound is not painful, but I experience sharp pain in the left scrotum.
Sometimes it feels like something is stuck in the blood vessels of the left scrotum, and then I suddenly feel blood flowing through.
I also feel this way when I sleep on my side, but not when I sleep on my back.
Are these symptoms normal after surgery? How long will it take for these symptoms to improve? Thank you.

Charles, 20~29 year old female. Ask Date: 2016/03/14

Post-varicocele surgery, it is not uncommon to experience discomfort and pain in the affected area, even if the surgical site itself feels fine. The symptoms you are describing, such as sharp pain in the left scrotum and sensations of something being stuck in the blood vessels, can be attributed to several factors related to the healing process after surgery.

Firstly, after a varicocele repair, the body undergoes a healing process that can involve inflammation and irritation of the surrounding tissues. This can lead to sensations of pain or discomfort in the scrotum, which may feel like a sharp or stabbing pain. The sensation of blood flow or a feeling of something being stuck can also be a result of the nerves in the area becoming more sensitive as they heal. This is known as neuropathic pain, which can occur after any surgical procedure.

In terms of recovery time, it is important to note that while the surgical site may heal relatively quickly, the surrounding tissues and nerves can take longer to fully recover. Generally, most patients begin to feel significant improvement within a few weeks post-surgery, but complete recovery can take several months. It is common for patients to experience varying levels of discomfort for up to 6 to 12 weeks after the procedure, especially during activities that involve movement or pressure on the area, such as sleeping positions or physical activity.

To manage your symptoms, here are a few recommendations:
1. Pain Management: If you are experiencing significant discomfort, consider discussing with your physician the possibility of adjusting your pain management plan. Over-the-counter pain relievers like ibuprofen or acetaminophen can help alleviate mild to moderate pain. If these are insufficient, your doctor may prescribe stronger medications.

2. Rest and Activity Modification: It's crucial to allow your body time to heal. Avoid strenuous activities or heavy lifting for at least a few weeks post-surgery. If certain positions, like side sleeping, exacerbate your pain, try to find a comfortable sleeping position that minimizes discomfort.

3. Follow-Up Appointments: Keep all scheduled follow-up appointments with your surgeon. They can monitor your healing process and address any concerns you may have. If your symptoms worsen or if you experience new symptoms, such as swelling, redness, or fever, seek medical attention promptly.

4. Ice Therapy: Applying ice packs to the area can help reduce inflammation and numb the pain. Just be sure to wrap the ice pack in a cloth to avoid direct contact with the skin.

5. Gentle Movement: While rest is important, gentle movement can also promote healing. Light walking can improve circulation and help reduce stiffness without putting too much strain on the surgical site.

In summary, while the sensations you are experiencing can be concerning, they are often part of the normal healing process after varicocele surgery. It is essential to communicate openly with your healthcare provider about your symptoms and follow their recommendations for recovery. With time and appropriate care, you should see improvement in your symptoms and overall recovery.
