Can root canal treatment be performed while wearing braces, or is it necessary to remove and reapply them?
This Saturday, I will be getting my upper braces.
After having two teeth extracted, I noticed that the fourth tooth on the left side was removed, and the fifth tooth, which had previously been filled with silver amalgam, now feels sore and painful when I bite down or eat.
Additionally, when I gently tap it with my fingertip, I can feel soreness in the gums as well.
I discussed this with my orthodontist, but the dentist explained that the X-ray only showed the filling and did not indicate any cavities.
I would like to ask other orthodontists: if I discover that there is a nerve issue requiring a root canal treatment after getting the braces, will it be possible to treat it? Or will I need to remove the braces first, undergo treatment, and then reapply the braces? Wouldn't that mean experiencing pain twice? The dentist I consulted believes the X-ray is fine and that no treatment is necessary.
I also visited a major hospital where the dentist suggested monitoring the situation, but if necessary, I might need a root canal treatment.
However, I am really worried and anxious since I am supposed to get my braces this Saturday.
I don't know what to do.

If pulpitis develops after braces have been placed, it is necessary to perform root canal treatment without removing the braces.
However, orthodontic treatment can continue.
The teeth that have already shown signs of pulp inflammation should undergo root canal therapy first to prevent apical infection due to pulpitis, which could affect the movement of the teeth during the orthodontic process.

When it comes to undergoing root canal treatment while wearing braces, there are several factors to consider. First and foremost, it is essential to understand the nature of your dental issue. If you are experiencing discomfort or pain in a tooth that has previously been treated with silver fillings, it may indicate an underlying problem, such as decay or nerve damage, that could necessitate root canal therapy.
In general, it is possible to perform root canal treatment on a tooth while you have braces on. However, the timing and coordination between your orthodontist and your endodontist (the specialist who performs root canal treatments) are crucial. If your orthodontist believes that the tooth in question is stable and does not require immediate treatment, they may recommend proceeding with the orthodontic treatment first. However, if there is a significant concern about the health of the tooth, it may be advisable to address the root canal issue before placing the braces.
One of the main concerns with waiting to treat a tooth that may need a root canal is the potential for further complications. If the tooth's nerve is indeed damaged, delaying treatment could lead to infection or abscess formation, which could complicate both the root canal procedure and the orthodontic treatment. In some cases, if the tooth becomes infected, it may require extraction, which would necessitate a different approach to your orthodontic treatment.
If you find yourself in a situation where you need root canal treatment after your braces have been placed, the procedure can still be performed. The orthodontic wires and brackets can be adjusted or temporarily removed to allow access to the tooth for treatment. After the root canal is completed, your orthodontist can reapply the braces. While this may seem like a cumbersome process, it is a common practice in dental care, and both your orthodontist and endodontist will work together to ensure the best outcome for your dental health.
In your case, since you are experiencing pain and discomfort, it would be prudent to seek a second opinion from a dental professional who specializes in endodontics. They can perform a thorough examination, including X-rays, to determine the exact condition of the tooth and whether root canal treatment is necessary. If the tooth is found to be in need of treatment, it may be best to address this before proceeding with braces to avoid any complications down the line.
In summary, while it is possible to undergo root canal treatment with braces on, careful consideration and communication between your dental care providers are essential. If there is any doubt about the health of your tooth, it is better to err on the side of caution and seek treatment before starting orthodontic work. Your dental health is paramount, and addressing issues like potential nerve damage early on can save you from more extensive treatments in the future.
