Anal pain during bowel movements?

Hello doctor, recently I have been experiencing a slight sharp pain in my anus during bowel movements, and there is a small amount of faint blood on the toilet paper after wiping.
After about three to four days, my bowel movements return to normal without pain.
This issue seems to improve for a while, but then it reoccurs.
What could be the cause of this?

LaLa, 20~29 year old female. Ask Date: 2016/03/24

Dr. Ke Fangxu reply Surgery

Based on your description, it seems to be an anal fissure.
The typical symptoms include pain during bowel movements accompanied by slight bleeding.
The causes of anal fissures are often due to hard stools, but diarrhea, frequent bowel movements, or prolonged straining can also lead to fissures.
It is recommended to improve bowel habits and to take warm sitz baths to alleviate symptoms.
Topical ointments and suppositories can also be used for treatment.
If symptoms persist for more than a week without improvement, it is advisable to consult a physician for further evaluation.

More Info

Anal pain and bleeding during bowel movements can be concerning symptoms, and it's important to understand the potential causes and appropriate management strategies. Based on your description, it sounds like you may be experiencing symptoms consistent with anal fissures or hemorrhoids, both of which are common conditions that can lead to pain and minor bleeding.

Possible Causes
1. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard or large stools. The pain associated with fissures is typically sharp and can be quite intense during and after bowel movements. The presence of blood on the toilet paper, especially if it is bright red, is a common sign of a fissure.

2. Hemorrhoids: These are swollen veins in the lower rectum or anus that can cause discomfort, itching, and bleeding. Hemorrhoids can be internal or external, and while they may not always cause pain, they can lead to bleeding during bowel movements, especially if there is straining involved.

3. Constipation: Straining during bowel movements due to constipation can exacerbate both fissures and hemorrhoids. Hard stools can cause trauma to the anal area, leading to pain and bleeding.

4. Dietary Factors: A diet low in fiber can contribute to constipation and the formation of hard stools. Insufficient hydration can also play a role in stool consistency.

Management Strategies
1. Dietary Changes: Increasing your intake of dietary fiber can help soften stools and promote regular bowel movements. Foods high in fiber include fruits, vegetables, whole grains, and legumes. Additionally, ensure you are drinking plenty of water throughout the day.

2. Regular Bowel Habits: Establishing a routine for bowel movements can help. Try to go at the same time each day, and don’t ignore the urge to go. Avoid straining during bowel movements, as this can worsen fissures and hemorrhoids.

3. Sitz Baths: Taking warm sitz baths can help soothe the anal area and promote healing. Soaking for about 10-15 minutes several times a day, especially after bowel movements, can provide relief.

4. Topical Treatments: Over-the-counter creams or ointments designed for hemorrhoids may provide relief from pain and discomfort. These can help reduce inflammation and promote healing. However, be cautious with any topical treatments and consult with a healthcare provider if you are unsure.

5. Avoiding Irritants: Be mindful of any products that may irritate the anal area, such as scented toilet paper or harsh soaps. Opt for gentle, unscented products to minimize irritation.

When to Seek Medical Attention
If your symptoms persist for more than a week, worsen, or if you notice significant bleeding, it is crucial to consult a healthcare professional. They can perform a thorough examination to rule out other potential causes of your symptoms, such as infections or more serious conditions.
In summary, while anal pain and minor bleeding during bowel movements can often be attributed to anal fissures or hemorrhoids, it is essential to address the underlying causes, such as constipation and dietary habits. Implementing lifestyle changes and seeking medical advice when necessary can help manage your symptoms effectively.
