Menstrual issues
Hello, Doctor: I have given birth to two children and underwent a tubal ligation surgery in July 2000.
Since then (up to July 2022), my menstrual cycle has been approximately 30 days.
However, I have been experiencing brown discharge before each period, followed by normal menstrual flow.
My last period started on July 7.
Around August 8, I had brown discharge again but did not have a normal period.
Instead, I noticed an increase in clear discharge.
I visited the obstetrics and gynecology clinic on August 18, and the results showed that I am not pregnant.
I also received a medication to induce menstruation.
The gynecologist mentioned that I have a hormonal imbalance.
As of August 20, I still have not had my period.
I would like to ask how Traditional Chinese Medicine would treat this condition.
Additionally, when and how should I use herbal formulas like Si Wu Tang and Zhong Jing Tang? Thank you for taking the time to consult with me.
Xiao Ling, 30~39 year old female. Ask Date: 2002/08/20
Dr. Chen Weili reply Traditional Chinese Medicine
Hi, Xiao Ling: Most gynecological conditions require a comprehensive evaluation and treatment in conjunction with other physiological symptoms.
It is advisable for you to seek medical attention nearby.
Additionally, I would like to provide the basic usage of Si Wu Tang and Zhong Jiang Tang.
Si Wu Tang can nourish and invigorate the blood, and it is recommended for individuals with a neutral constitution to take it after menstruation has cleared, approximately 2 to 4 doses per month for general conditioning.
If you experience dysmenorrhea, gastrointestinal sensitivity, irregular menstruation, or symptoms related to menopause, and have a tendency towards a cold constitution, it is recommended to use Zhong Jiang Tang for conditioning.
During menstruation, aside from dysmenorrhea, please avoid using it.
The key point is that both are for conditioning purposes, and if there are any health issues, you still need to see a doctor.

Dear Patient,
Thank you for reaching out with your concerns regarding menstrual issues and the potential role of Traditional Chinese Medicine (TCM) in addressing them. It’s important to understand that menstrual irregularities can stem from various factors, including hormonal imbalances, stress, lifestyle changes, and underlying health conditions. Given your history of two pregnancies and a tubal ligation, it’s understandable that you may experience changes in your menstrual cycle.
In TCM, the menstrual cycle is closely linked to the balance of Qi (energy), Blood, and the health of the organs involved in reproduction, particularly the Liver, Spleen, and Kidneys. The presence of brown discharge before menstruation can indicate a few things, such as Blood stagnation or a deficiency in Blood or Qi. This is often interpreted as a sign that the body is not effectively moving Blood, which can lead to irregularities in the menstrual cycle.
TCM Approaches to Treating Menstrual Issues
1. Diagnosis: A TCM practitioner would typically conduct a thorough assessment, including a detailed inquiry about your menstrual history, lifestyle, diet, and emotional well-being. This holistic approach helps to identify the root cause of your symptoms.
2. Herbal Medicine: Based on the diagnosis, a TCM practitioner may prescribe herbal formulas tailored to your specific needs. Common formulas for menstrual issues include:
- Si Wu Tang (四物湯): This is a classic formula used to nourish Blood and regulate menstruation. It is often recommended for women experiencing Blood deficiency, which can manifest as irregular periods or light menstrual flow.
- Zheng Gu Shui (正骨水): While primarily used for pain relief, it can also help with Blood circulation.
- Gui Pi Tang (歸脾湯): This formula is beneficial for those with Spleen Qi deficiency, which can lead to fatigue and irregular menstruation.
3. Timing of Use: The timing of when to take these herbal formulas can be crucial. For instance, Si Wu Tang is often recommended to be taken in the days leading up to menstruation to help prepare the body for a healthy cycle. It’s essential to consult with a qualified TCM practitioner to determine the best timing based on your specific situation.
4. Acupuncture: Acupuncture can also be an effective treatment for menstrual irregularities. It helps to balance the body's energy, improve circulation, and regulate hormonal levels. Specific acupuncture points may be targeted to enhance Blood flow and alleviate symptoms.
5. Lifestyle and Dietary Recommendations: TCM emphasizes the importance of a balanced diet and lifestyle. Incorporating foods that nourish Blood, such as dark leafy greens, red meats, and legumes, can be beneficial. Additionally, managing stress through practices like Tai Chi or meditation can support overall hormonal balance.
Conclusion
In summary, TCM offers a holistic approach to treating menstrual issues by focusing on restoring balance within the body. It is crucial to work with a qualified TCM practitioner who can provide personalized treatment based on your unique health profile. They will guide you on the appropriate use of herbal formulas like Si Wu Tang and any necessary lifestyle adjustments.
If you have further questions or need assistance finding a TCM practitioner, please feel free to reach out. Your health and well-being are of utmost importance, and I wish you the best on your journey to restoring balance in your menstrual cycle.
