Wrist discomfort?
Hello, Dr.
Shi.
I have been suffering from a wrist condition for almost a month.
The symptoms include pain on the ulnar side of my left wrist when I rotate it outward in a clockwise direction.
Additionally, when I carry a tray or a book (with my palm facing up and applying weight on the palm), I also experience pain on the ulnar side of my wrist.
I visited a major hospital, and the doctor diagnosed me with triangular fibrocartilage complex (TFCC) inflammation.
X-rays showed no abnormalities in the bony structures, but an ultrasound indicated tendonitis with signs of fibrosis.
(Hearing about fibrosis makes me feel quite concerned.) I have been taking anti-inflammatory medication and muscle relaxants after every meal and have been undergoing rehabilitation (electrotherapy and localized heat ultrasound therapy) for two weeks now, but there has been no noticeable improvement.
I would like to ask you, Dr.
Shi, whether my current condition is severe enough that it cannot be cured, or what treatment options might lead to better improvement.
Thank you.

Dr. Shi Guozheng reply Orthopedics
Symptoms of this nature typically take about six weeks to resolve, so continued treatment is necessary.
Additionally, it is important not to exert too much force on the affected joint.
Inflammation of the wrist can be difficult to heal, but with patience in treatment, improvement will occur.

More Info
Hello! It sounds like you are dealing with a frustrating wrist condition, and I appreciate you sharing your symptoms and treatment history. Based on your description, it appears that you are experiencing pain on the ulnar side of your left wrist, particularly during certain movements and when carrying weight. The diagnosis of triangular fibrocartilage complex (TFCC) inflammation and tendonitis, along with the mention of fibrosis, suggests that there may be some chronic changes occurring in the soft tissues of your wrist.
Understanding Your Condition
The triangular fibrocartilage complex (TFCC) is a structure in the wrist that plays a crucial role in stabilizing the joint, particularly during wrist movements that involve rotation or loading. Inflammation of the TFCC can lead to pain, especially when performing activities that put stress on the wrist, such as lifting or twisting motions. The presence of tendonitis indicates that the tendons around the wrist may also be inflamed, which can contribute to your discomfort.
Treatment Options
1. Medication: It seems you are already on anti-inflammatory medications and muscle relaxants. These can help reduce pain and inflammation, but they may not address the underlying issues if the inflammation is chronic.
2. Physical Therapy: You mentioned that you are undergoing rehabilitation with modalities like electrical stimulation and ultrasound therapy. These treatments can be beneficial for reducing pain and promoting healing. However, it may be helpful to incorporate specific exercises that focus on strengthening the wrist and improving flexibility. A physical therapist can guide you through exercises tailored to your condition.
3. Activity Modification: It’s essential to avoid activities that exacerbate your symptoms. If lifting heavy objects or certain wrist movements cause pain, try to modify these activities or use assistive devices to reduce strain on your wrist.
4. Injections: If conservative treatments do not provide relief, corticosteroid injections may be considered. These can help reduce inflammation in the affected area. However, repeated injections should be approached with caution due to potential side effects.
5. Surgery: In cases where conservative treatments fail and the pain significantly impacts daily activities, surgical options may be explored. This could involve repairing the TFCC or addressing any tendon issues. A consultation with an orthopedic surgeon specializing in wrist injuries may provide more insight into this option.
Recovery Tips
- Rest: Allow your wrist to rest and avoid activities that cause pain. This is crucial for healing.
- Ice Therapy: Applying ice to the affected area can help reduce inflammation and pain. Aim for 15-20 minutes several times a day.
- Gradual Return to Activity: Once your symptoms improve, gradually reintroduce activities, ensuring you do not overexert your wrist.
- Strengthening Exercises: Once the acute pain subsides, focus on strengthening exercises for the wrist and forearm to prevent future injuries.
Prognosis
The severity of your condition and the potential for recovery depend on various factors, including the extent of the inflammation, any underlying structural issues, and how well you respond to treatment. While some cases of TFCC inflammation can resolve with conservative management, others may require more intensive interventions.
It's essential to maintain open communication with your healthcare providers and to follow their recommendations closely. If you feel that your current treatment plan is not yielding results, don't hesitate to seek a second opinion or ask for a referral to a specialist who can provide a more tailored approach to your condition.
In conclusion, while your symptoms may feel discouraging, many patients with similar conditions can achieve significant improvement with the right combination of treatments and lifestyle modifications. Stay proactive in your care, and I wish you the best in your recovery journey!
