Discomfort in the urethra?

Hello Doctor: I have been experiencing an uncomfortable sensation in my penile urethra.
It feels somewhat like a burning sensation, but not exactly.
It also feels like I have the urge to urinate, along with a slight feeling of urinary obstruction.
However, I had a urine test at the urology department, and there were no issues.
Could you please tell me what this problem might be? I really dislike this sensation, as I have never felt this way before.

Mr. Huang, 20~29 year old female. Ask Date: 2016/04/01

Dr. Li Xinyi reply Urology

It sounds like a challenging issue.
I recommend starting with proper hygiene: 1.
Avoid wearing tight and warm pants.
2.
It is advisable to wash only with water and avoid any contact of the glans with chemical substances.
Try this for a period of time.
3.
After showering, use a hairdryer to dry the area.
4.
Make sure to dry off after urinating.

Understanding discomfort in the urethra can be quite complex, as it can stem from various underlying causes. Based on your description of a burning sensation, a feeling of urgency, and a sensation of obstruction in the urethra, it is essential to consider several potential factors that could be contributing to your discomfort.

1. Urethritis: This is an inflammation of the urethra, often caused by infections, including sexually transmitted infections (STIs) such as chlamydia or gonorrhea. Even if your urine tests came back normal, it is possible to have urethritis without significant findings in a urinalysis. Symptoms can include burning during urination, increased frequency of urination, and discomfort in the urethra.

2. Prostatitis: In men, inflammation of the prostate gland can lead to discomfort in the urethra. Chronic prostatitis can sometimes present with vague symptoms, including pelvic pain, discomfort during urination, and a feeling of fullness or pressure in the urethra. A thorough evaluation by a urologist, including a digital rectal exam, may be necessary to assess for prostatitis.

3. Interstitial Cystitis: This is a chronic condition characterized by bladder pressure, bladder pain, and sometimes pelvic pain. It can cause symptoms similar to a urinary tract infection (UTI) without the presence of an infection. Patients often experience urgency and frequency of urination, along with discomfort in the urethra.

4. Nerve-related Issues: Sometimes, discomfort in the urethra can be related to nerve irritation or damage. Conditions such as pelvic floor dysfunction or nerve entrapment can lead to sensations of burning or pressure in the urethra.

5. Allergic Reactions or Irritants: Exposure to certain soaps, lotions, or laundry detergents can cause irritation of the genital area and urethra. If you have recently changed any personal care products, consider whether they could be contributing to your symptoms.

6. Psychological Factors: Anxiety and stress can sometimes manifest as physical symptoms, including discomfort in the urethra. If you have been experiencing increased stress or anxiety, it may be worth exploring this aspect with a healthcare professional.

Given that your initial tests did not reveal any significant issues, it may be beneficial to follow up with a urologist for further evaluation. They may recommend additional tests, such as a cystoscopy, to visually inspect the urethra and bladder for any abnormalities.
In the meantime, consider the following self-care measures:
- Stay Hydrated: Drinking plenty of water can help dilute your urine and may alleviate some discomfort.

- Avoid Irritants: Steer clear of potential irritants such as caffeine, alcohol, spicy foods, and acidic foods, which can exacerbate urinary symptoms.

- Warm Baths: Soaking in a warm bath may help relieve discomfort temporarily.

In conclusion, while your symptoms are understandably concerning, there are various potential causes for your discomfort in the urethra. A thorough evaluation by a healthcare professional, particularly a urologist, will be essential in determining the underlying cause and developing an appropriate treatment plan. Don't hesitate to seek further medical advice to ensure your symptoms are properly addressed.
