Glans penis abrasion?

Hello doctor, I had sexual intercourse about two weeks ago, during which I used a condom.
However, I noticed a red spot on the glans that feels a bit like a abrasion.
Since I rode a motorcycle that day, I'm unsure if the friction from my underwear caused the irritation or if it was due to the sexual activity, but it has been almost two weeks since the last intercourse.
My urination has been normal, with no burning sensation or any other issues, but the area with the abrasion is painful to touch, and I noticed some pus while showering.
I suspect it may not be a sexually transmitted infection.
Since there have been no outpatient services at hospitals or clinics due to the recent holidays, I have been applying a topical antibiotic ointment.
Is this appropriate? Thank you.

Hemo, 20~29 year old female. Ask Date: 2016/04/03

Dr. Li Xinyi reply Urology

Do not apply ointment from a nurse if it is a sexually transmitted disease.
If you have concerns, it is difficult to provide an answer.
If there has been risky sexual behavior, it is advisable to see a urologist for examination and follow-up.

Injuries to the glans, or the head of the penis, can occur for various reasons, including friction, trauma, or infections. Understanding the causes, symptoms, and treatment options is crucial for proper management and recovery.

Causes of Glans Injury
1. Friction or Trauma: As you mentioned, activities such as vigorous sexual intercourse or even riding a bicycle can lead to friction injuries. The constant rubbing against clothing or during sexual activity can cause irritation or abrasions on the sensitive skin of the glans.

2. Infections: While you noted that you have not experienced any urinary symptoms such as pain during urination, it is still possible for infections to occur. Conditions like balanitis (inflammation of the glans) can arise from irritants, poor hygiene, or infections, including sexually transmitted infections (STIs).

3. Allergic Reactions: Sometimes, the materials in condoms or lubricants can cause allergic reactions, leading to redness and irritation.

Symptoms of Glans Injury
The symptoms you described, such as redness, a sensation similar to a scrape, and localized pain, are common indicators of a minor injury. The presence of pus or discharge may suggest an infection, which requires further evaluation. Other symptoms to watch for include:
- Swelling or inflammation
- Itching or burning sensations
- Discoloration or unusual spots on the glans
- Pain during sexual activity or when touching the area

Treatment Options
1. Self-Care: For minor injuries, self-care is often sufficient. This includes:
- Keeping the Area Clean: Gently wash the area with mild soap and water. Avoid harsh soaps or chemicals that could further irritate the skin.

- Avoiding Irritants: Refrain from using any products that may cause irritation, such as scented soaps or lotions.

- Topical Treatments: Applying a mild antiseptic ointment can help prevent infection. However, be cautious with over-the-counter creams, as some may not be suitable for sensitive areas.

2. Rest and Avoidance: It’s essential to avoid further irritation. This means refraining from sexual activity until the area has healed. If friction from clothing is a concern, consider wearing looser-fitting underwear.

3. Monitoring Symptoms: Keep an eye on the injury. If the redness, pain, or any discharge worsens, or if you develop additional symptoms such as fever or systemic signs of infection, it is crucial to seek medical attention.

4. Consulting a Healthcare Provider: If the injury does not improve within a few days or if you notice signs of infection (such as increased redness, swelling, or pus), it is important to consult a healthcare provider. They may perform a physical examination and, if necessary, prescribe antibiotics or other treatments.

Conclusion
While it is understandable to be concerned about the possibility of a sexually transmitted infection, the symptoms you describe are more consistent with a minor friction injury or irritation. However, given the potential for complications, especially if there is any sign of infection, it is advisable to seek medical attention if symptoms persist or worsen. In the meantime, practicing good hygiene and avoiding further irritation will aid in your recovery. If you have any doubts or if the condition does not improve, do not hesitate to reach out to a healthcare professional for a thorough evaluation.
