Glans penis abrasion?
Hello, Dr.
Hsu.
Yesterday, while I was washing my husband's penis, I’m not sure if it was because I washed for too long or if I was too rough (I really thought I was being gentle >"<), but halfway through, I noticed that his glans was bleeding.
Upon closer inspection, I found seven or eight small red spots on the glans that looked like tiny holes (smaller than a grain of rice), and one of them was bleeding a little bit.
What kind of medication can be applied in this situation? Also, what does this condition indicate? Additionally, I have not had sexual intercourse for over a month due to vaginal inflammation and have been applying medication, but I perform oral sex on my husband every 2 to 3 days.
Could this be a contributing factor? I would appreciate your guidance.
Thank you! ^^

Hello, it is possible that the localized skin breakdown is caused by an infection.
It may also be due to excessive cleaning.
Generally, cleaning the foreskin can be done with cold water only, without the use of any cleaning products or hot water.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
When it comes to understanding glans injury, it is essential to recognize the potential causes, symptoms, and treatment options available. Based on your description, it appears that your husband has experienced some form of trauma to the glans (the head of the penis), which has resulted in bleeding and the appearance of small red spots that resemble tiny punctures or abrasions.
Causes of Glans Injury
1. Mechanical Trauma: This is the most common cause of injury to the glans. It can occur due to vigorous washing, sexual activity, or even friction from clothing. In your case, it seems that the washing might have been too aggressive, leading to minor abrasions.
2. Infections: Conditions such as balanitis (inflammation of the glans) can cause redness, swelling, and sometimes bleeding. If there is an underlying infection, it could exacerbate the symptoms.
3. Allergic Reactions: Sometimes, the use of soaps, lotions, or other products can cause irritation or allergic reactions, leading to redness and discomfort.
4. Sexually Transmitted Infections (STIs): Certain STIs can cause lesions or sores on the glans, although this is less likely if there are no other symptoms present.
Symptoms
The symptoms you described—small red dots and bleeding—are indicative of minor trauma or irritation. The presence of bleeding, even if slight, suggests that the skin has been compromised. Other symptoms to watch for include:
- Swelling or inflammation
- Pain or tenderness
- Discharge (which may indicate infection)
- Persistent redness or irritation
Treatment Options
1. Topical Treatments: For minor abrasions, applying a gentle antiseptic ointment (like Neosporin) can help prevent infection and promote healing. Avoid using alcohol-based products, as they can cause further irritation.
2. Avoid Irritation: It is crucial to avoid any further irritation to the area. This means being gentle during washing and avoiding any activities that could exacerbate the injury, such as vigorous sexual activity until healing occurs.
3. Hygiene: Maintain good hygiene by gently cleaning the area with mild soap and water. Avoid harsh soaps or products that could irritate the skin.
4. Consult a Healthcare Provider: If the bleeding persists, the spots do not improve, or if other symptoms develop (such as fever, increased pain, or discharge), it is essential to consult a healthcare provider. They may perform a physical examination and, if necessary, run tests to rule out infections or other underlying conditions.
Impact of Oral Sex
Regarding your concern about performing oral sex while treating a vaginal infection, it is generally advisable to avoid sexual activities, including oral sex, until the infection has fully resolved. This is to prevent the potential transfer of bacteria or irritants that could worsen either partner's condition. If your husband is experiencing irritation or injury, it would be best to refrain from any sexual activity until he has healed completely.
Conclusion
In summary, your husband's symptoms likely stem from minor trauma due to washing. The best course of action is to apply a gentle antiseptic, avoid further irritation, and maintain good hygiene. If symptoms persist or worsen, seeking medical attention is crucial. Additionally, it is wise to avoid sexual activities until both partners are free from any infections or injuries to ensure a safe and healthy experience.
