Is this menstrual cycle normal?
Hello, doctor.
My menstrual period has always lasted 7 days with a cycle of 28-32 days.
I usually have a relatively heavy flow and do not experience significant menstrual pain.
On average, I use about 8 pads a day, especially during the first 4 days when I definitely need to use 41 cm sanitary pads.
However, in my recent menstrual cycles, the flow has been unusual.
For the first 1-2 days, there is some blood, but it's not much.
The flow becomes heavier around days 4-5, and it usually takes until the 8th day for everything to clear up.
Sometimes, the flow is heavy on days 1-3, but lighter later on, with some brown discharge, and it also takes until the 8th day to fully resolve.
I am wondering if this could indicate any gynecological issues.
Thank you, doctor.

Hello, the causes of abnormal menstrual bleeding in middle-aged women may include hormonal imbalances, emotional factors, diet, and age, among others.
However, it is crucial to rule out conditions such as uterine tumors, endometrial hyperplasia, cervical lesions, and vaginal lesions.
If the abnormal bleeding persists, it is advisable to consult a physician for further evaluation.
Thank you! Wishing you peace and health.

Understanding menstrual cycle irregularities is crucial for women's health, as it can provide insights into underlying conditions and overall reproductive health. Based on your description, your menstrual cycle has generally been consistent, lasting 7 days with a cycle length of 28-32 days. However, recent changes in your menstrual flow—specifically, a lighter flow in the first few days followed by heavier bleeding, and the presence of brown discharge—can raise concerns.
Firstly, it's important to note that variations in menstrual flow can occur due to several factors, including hormonal fluctuations, stress, changes in weight, exercise habits, and even diet. Hormonal imbalances, particularly involving estrogen and progesterone, can lead to changes in the menstrual cycle. For instance, if ovulation is irregular, it can result in lighter periods or spotting before the actual flow begins.
The presence of brown discharge, especially towards the end of your period, is typically old blood that takes longer to exit the body. This can be normal, especially if it occurs at the tail end of your menstrual cycle. However, if this pattern persists or is accompanied by other symptoms such as severe pain, unusual odor, or significant changes in your cycle, it may warrant further investigation.
Given that you mentioned using a significant amount of sanitary products (8 pads a day) during your heaviest days, it suggests that you experience a relatively heavy flow, which is medically termed menorrhagia. While heavy menstrual bleeding can be normal for some women, it can also indicate underlying conditions such as fibroids, polyps, or hormonal imbalances.
It is also worth considering other factors that could influence your menstrual cycle. For example, stress and lifestyle changes can impact hormonal balance. Additionally, conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can lead to irregularities in menstrual cycles.
If you are experiencing significant changes in your menstrual pattern, it is advisable to consult with a healthcare provider. They may recommend a thorough evaluation, including a physical examination, blood tests to check hormone levels, and possibly imaging studies like an ultrasound to assess for any structural abnormalities in the reproductive organs.
In summary, while some variations in menstrual cycles are normal, the changes you are experiencing—especially if they are new and persistent—should be discussed with a healthcare professional. They can provide personalized advice and determine if any further investigation or treatment is necessary. Keeping a menstrual diary, noting the flow, duration, and any accompanying symptoms, can also be helpful for your healthcare provider in understanding your situation better.
