Bright red blood in stool?

Hello, Dr.
Ke: Recently, I've been under a lot of stress from my business, resulting in insufficient sleep.
I often eat spicy food and used to drink alcohol frequently.
Today, I had a bowel movement four to five times, and during the last one, I noticed a lot of blood, which was bright red when I dabbed it with tissue.
This has happened a few days ago as well.
For the past few years, I have experienced urgent bowel movements right after meals, and they are often loose, resembling diarrhea.
Sometimes, there is a thick, sticky, yellow jelly-like substance.
When I consulted doctors, they suggested it might be irritable bowel syndrome.
I'm unsure if this is related to my high stress levels and excessive heat in my body, or if there is a real health issue.
Thank you!

Jie, 30~39 year old female. Ask Date: 2016/04/14

Based on your description, it seems more like irritable bowel syndrome combined with hemorrhoidal bleeding.
It is recommended to avoid constipation during this period and to shorten the time spent on bowel movements.
Warm sitz baths should also help alleviate symptoms.
You can purchase over-the-counter hemorrhoidal suppositories for use.
It is best not to take stool softeners indiscriminately; at this age, increasing dietary fiber and water intake should be sufficient.
If symptoms persist without improvement, you may consult a physician for medication.
If there is still no improvement or if you are very concerned, a colonoscopy can be arranged.

When it comes to the presence of fresh blood in stool, it is essential to understand the potential causes and the implications for your health. Fresh blood in the stool can be alarming and may indicate various underlying conditions, some of which require immediate medical attention.

Given your description, several factors could be contributing to your symptoms. The stress from your business, lack of sleep, and dietary choices (such as consuming spicy foods and alcohol) can all impact your gastrointestinal health. These lifestyle factors can lead to conditions like irritable bowel syndrome (IBS), which you mentioned has been suggested by your doctors. IBS is characterized by symptoms such as abdominal pain, bloating, and changes in bowel habits, including diarrhea or constipation. It can be exacerbated by stress and certain foods.

However, the presence of fresh red blood in your stool is a symptom that should not be overlooked. Fresh blood typically indicates bleeding from the lower gastrointestinal tract, which could be due to several reasons:
1. Hemorrhoids: Swollen veins in the rectum or anus that can bleed during bowel movements, especially if you are straining or have constipation.

2. Anal fissures: Small tears in the lining of the anus, often caused by passing hard stools, which can lead to bleeding and pain.

3. Diverticular disease: This includes diverticulosis and diverticulitis, where small pouches form in the colon wall and can become inflamed or infected, leading to bleeding.

4. Colorectal polyps or cancer: Polyps are growths on the lining of the colon that can bleed. Some polyps can become cancerous over time, which is why any bleeding should be evaluated.

5. Inflammatory bowel disease (IBD): Conditions like Crohn's disease or ulcerative colitis can cause inflammation in the gastrointestinal tract, leading to symptoms like diarrhea, abdominal pain, and blood in the stool.

Given the recurrent nature of your symptoms and the presence of fresh blood, it is crucial to seek further evaluation from a healthcare professional. A gastroenterologist would be the appropriate specialist to consult, as they can perform necessary diagnostic tests, such as a colonoscopy, to visualize the colon and identify any abnormalities.

In the meantime, consider making some lifestyle changes to help manage your symptoms. Reducing stress through relaxation techniques, ensuring adequate sleep, and modifying your diet to include more fiber while avoiding irritants like spicy foods and alcohol may help alleviate some gastrointestinal discomfort. Staying hydrated is also essential, especially if you are experiencing diarrhea.

In conclusion, while stress and dietary habits can contribute to gastrointestinal issues, the presence of fresh blood in your stool is a significant symptom that warrants further investigation. Please prioritize scheduling an appointment with a gastroenterologist to ensure a thorough evaluation and appropriate management of your symptoms. Your health is paramount, and addressing these concerns early can lead to better outcomes.
