Hair loss, irregular menstruation, weight gain, and easy fatigue?

Hello, Doctor: I would like to ask you about my condition.
I started experiencing hair loss around July of last year and have visited 3-4 clinics and one large hospital for examinations.
The diagnosis from each place has been "alopecia areata," which is said to be untreatable.
I have been prescribed topical steroids to apply, as I am hesitant to receive injections in my scalp.

Since around October of last year, my menstrual cycle has become irregular.
Previously, I had a consistent cycle of 32 days, but now my periods are delayed by 7 to 15 days.
Although I have been exercising less, I am certain that I do not overeat; my meals are normal with a balance of vegetables and meat, yet I have gained weight rapidly—about 5 kilograms in the past six months.

I usually go to bed around 11:30 PM and wake up at 7:00 AM on weekdays, and on weekends, I go to bed around 11:30 PM and wake up at 8:00 AM, which I consider to be a normal routine.
Sometimes, I feel dizzy after light exercise, or I struggle to catch my breath, and my overall energy levels have significantly decreased.

After researching online, I found that others with similar issues suggested that it might be due to hormonal imbalance.
Therefore, I would like to ask you, Doctor, is my situation indeed related to hormonal imbalance? Should I get a blood test? Is it sufficient to see a general internist? I have also seen recommendations online to consult a traditional Chinese medicine practitioner for better health, but I would like to identify the underlying cause first before making a decision.
Thank you.

Hello: It is recommended that you consult the "Endocrinology and Metabolism Department" as misdiagnosis can occur if the wrong specialty is chosen.

More Info

Your situation involves multiple symptoms that could be interconnected, including hair loss, irregular menstrual cycles, weight gain, and fatigue. These issues can indeed be indicative of an underlying hormonal imbalance or other medical conditions. Let's break down your concerns and explore potential causes and solutions.

Hair Loss (Alopecia Areata)
You mentioned that you have been diagnosed with "alopecia areata," which is an autoimmune condition that leads to hair loss in patches. While there is no definitive cure, treatments such as topical corticosteroids can help reduce inflammation and promote hair regrowth. It's important to follow your dermatologist's advice and continue with the prescribed treatment. If you're uncomfortable with injections, topical treatments are a valid alternative, although they may take longer to show results.

Irregular Menstrual Cycles
The change in your menstrual cycle from a regular 32-day cycle to delays of 7-15 days can be concerning. Irregular periods can be caused by various factors, including stress, hormonal imbalances, weight fluctuations, and lifestyle changes. Given that you have experienced weight gain and fatigue, it's possible that these factors are contributing to your menstrual irregularities. Conditions such as polycystic ovary syndrome (PCOS) are known to cause both irregular periods and weight gain, and they often accompany symptoms like hair loss.

Weight Gain and Fatigue
You noted a weight gain of 5 kilograms over the past six months, despite maintaining a normal diet and sleep schedule. This unexpected weight gain, combined with fatigue and feelings of dizziness during exercise, could suggest a metabolic or hormonal issue. Conditions such as hypothyroidism or insulin resistance can lead to weight gain and fatigue, and they can also affect your menstrual cycle.

Possible Hormonal Imbalance
Given the combination of symptoms—hair loss, irregular periods, weight gain, and fatigue—it is reasonable to suspect a hormonal imbalance. Hormones such as estrogen, progesterone, and testosterone, as well as thyroid hormones, play crucial roles in regulating these bodily functions. An imbalance can lead to the symptoms you are experiencing.

Recommendations
1. Consult a Healthcare Provider: It is advisable to see a healthcare provider, preferably an endocrinologist, who specializes in hormonal issues. They can conduct a thorough evaluation, including blood tests to check hormone levels, thyroid function, and other relevant markers.

2. Blood Tests: Request tests for thyroid hormones (TSH, Free T4, Free T3), sex hormones (estrogen, progesterone, testosterone), and possibly insulin levels. These tests can help identify any underlying conditions contributing to your symptoms.

3. Lifestyle Modifications: While you mentioned a normal diet, consider consulting a nutritionist to ensure you are meeting your nutritional needs, especially if you are experiencing fatigue. Regular physical activity, even light exercise, can help improve your overall well-being and may assist in regulating your menstrual cycle.

4. Stress Management: Since stress can exacerbate hormonal imbalances, incorporating stress-reducing practices such as mindfulness, yoga, or meditation may be beneficial.

5. Consider Alternative Therapies: If you are open to it, consulting with a licensed acupuncturist or a practitioner of traditional Chinese medicine may provide additional support for balancing your hormones and improving your overall health.

6. Follow-Up: Keep track of your symptoms and any changes you notice after implementing these recommendations. Regular follow-ups with your healthcare provider will be essential to monitor your progress and adjust treatment as necessary.

In conclusion, your symptoms are likely interconnected and may be indicative of a hormonal imbalance. A comprehensive evaluation by a healthcare professional is essential to identify the underlying causes and develop an appropriate treatment plan. Remember, addressing these issues holistically can lead to significant improvements in your overall health and well-being.
