Nipple discharge

Hello Doctor: A few months ago, I noticed something like a pimple on the edge of my nipple.
When I squeezed it, a soft, white, string-like substance came out.
There is no such issue on the left side.
Could you please tell me what this problem might be? Thank you.

JJ, 20~29 year old female. Ask Date: 2016/04/18

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear J J: There may be a condition of skin cysts, such as sebaceous cysts.
If it is caused by ductal secretions, it is a benign change that can be monitored.
If symptoms persist, a consultation in the outpatient clinic is recommended.

More Info

Nipple discharge can be a concerning symptom for many individuals, and understanding its causes is crucial for maintaining breast health. Based on your description, it seems you have noticed a white, soft, string-like discharge from one nipple, resembling a pimple or a clogged pore. This can be attributed to several factors, and I will outline some potential causes and considerations regarding nipple discharge.

Common Causes of Nipple Discharge
1. Duct Ectasia: This condition occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens. It can lead to a sticky discharge, often dark or greenish, but can also present as white. This is more common in women approaching menopause.

2. Hormonal Changes: Fluctuations in hormones, particularly estrogen and progesterone, can lead to changes in breast tissue and discharge. This is often seen in relation to menstrual cycles or hormonal therapies.

3. Infections: Infections of the breast tissue or ducts can lead to discharge. If the discharge is accompanied by redness, swelling, or pain, it may indicate an infection that requires medical attention.

4. Fibrocystic Changes: Many women experience fibrocystic breast changes, which can cause lumps and discharge. These changes are often benign but can cause discomfort and require monitoring.

5. Papillomas: These are benign tumors that can develop in the ducts of the breast and may cause discharge. They can sometimes be mistaken for more serious conditions, so evaluation by a healthcare provider is recommended.

6. Breast Cancer: While less common, certain types of breast cancer can present with nipple discharge. This is typically unilateral (affecting one breast) and may be accompanied by other symptoms such as a lump, changes in breast shape, or skin changes.

When to Seek Medical Attention
It is essential to consult with a healthcare provider if you notice any of the following:
- Discharge that is bloody or clear and sticky.

- Discharge that occurs spontaneously without squeezing the nipple.

- Accompanying symptoms such as breast pain, swelling, or changes in the skin.

- A lump or mass in the breast.

Diagnostic Approach
Your healthcare provider may recommend several diagnostic tests to determine the cause of the discharge:
- Clinical Breast Exam: A thorough examination of the breast tissue.

- Ultrasound or Mammography: Imaging studies to assess for any underlying abnormalities.

- Ductography: A specialized imaging technique that can visualize the ducts of the breast.

- Biopsy: If there are concerning findings, a biopsy may be necessary to rule out malignancy.

Conclusion
In summary, while nipple discharge can be benign and related to hormonal changes or duct issues, it is crucial to monitor the characteristics of the discharge and any accompanying symptoms. Given that you have noticed this issue only on one side, it is advisable to seek a professional evaluation to ensure that any potential underlying conditions are addressed promptly. Regular breast self-exams and awareness of changes in your breast health are vital components of maintaining overall breast health.
