The effects of saw palmetto on prostate health?
Hello Doctor, I am 30 years old and I am not sure why I experience a feeling of soreness and heaviness in the perineum after prolonged erections or after ejaculation.
During these times, I also have difficulty urinating, although I feel fine otherwise.
I have started doing pelvic floor exercises and taking warm sitz baths.
I have also heard that saw palmetto is better for the prostate than pumpkin seed extract.
What is your opinion on this? Additionally, does saw palmetto have any side effects? Thank you.
Niming, 30~39 year old female. Ask Date: 2016/04/27
Dr. Xu Wencang reply Urology
Health supplements can be tried, but outpatient examination and treatment are still necessary.

More Info
Saw palmetto, derived from the fruit of the Serenoa repens plant, has gained popularity as a natural supplement for promoting prostate health, particularly in the context of benign prostatic hyperplasia (BPH). As a 30-year-old experiencing discomfort in the perineal area and urinary difficulties after prolonged erections or ejaculation, it’s understandable to seek alternative treatments that may alleviate these symptoms.
Benefits of Saw Palmetto
1. Prostate Health: Saw palmetto is often used to manage symptoms associated with BPH, such as frequent urination, weak urine flow, and the sensation of incomplete bladder emptying. Some studies suggest that saw palmetto may help reduce the size of the prostate and improve urinary function, although results can vary among individuals.
2. Hormonal Balance: Saw palmetto is believed to inhibit the conversion of testosterone to dihydrotestosterone (DHT), a hormone implicated in prostate enlargement. By potentially lowering DHT levels, saw palmetto may help mitigate some of the symptoms associated with prostate enlargement.
3. Anti-inflammatory Properties: The anti-inflammatory effects of saw palmetto may contribute to its benefits for prostate health, as inflammation is often a contributing factor in prostate-related issues.
Side Effects of Saw Palmetto
While saw palmetto is generally considered safe for most individuals, it can have side effects. Some of the reported side effects include:
1. Gastrointestinal Issues: Some users may experience stomach upset, nausea, diarrhea, or constipation. These symptoms are usually mild and can be managed by taking the supplement with food.
2. Hormonal Effects: Since saw palmetto affects hormone levels, it may lead to changes in libido, erectile function, or breast tenderness in some men. It is essential to monitor any changes in sexual health when starting this supplement.
3. Bleeding Risks: Saw palmetto may have blood-thinning effects, which could increase the risk of bleeding, especially if taken with other anticoagulant medications. It’s crucial to discuss this with a healthcare provider if you are on blood thinners.
4. Allergic Reactions: Though rare, some individuals may experience allergic reactions to saw palmetto, which could manifest as skin rashes or respiratory issues.
Considerations for Your Symptoms
Given your symptoms of perineal discomfort and urinary difficulties, it’s commendable that you are engaging in pelvic floor exercises and warm sitz baths, which can be beneficial for pelvic health. However, before starting saw palmetto or any new supplement, it’s advisable to consult with a healthcare provider, particularly a urologist. They can evaluate your symptoms in the context of your overall health and determine if saw palmetto is appropriate for you.
Conclusion
Saw palmetto may offer potential benefits for prostate health, particularly in managing symptoms of BPH. However, it is essential to weigh these benefits against the possible side effects and to consider individual health circumstances. Regular follow-ups with your healthcare provider can help monitor your symptoms and adjust your treatment plan as necessary. Always prioritize open communication with your healthcare team regarding any supplements you wish to incorporate into your regimen.
