Can the body feel the heartbeat?
Hello Doctor, I often feel my body pulsating in rhythm with my heartbeat, especially when lying down, and it becomes more pronounced.
It feels like my entire chest is vibrating, and I get "shaken awake" just before falling asleep.
I have had an echocardiogram that showed mild mitral valve prolapse, a 24-hour Holter monitor that was normal, and thyroid tests that were also fine.
Could you please tell me what might be the issue? Thank you, Doctor!

Feeling your heartbeat in a pronounced manner, especially when lying down, can be a disconcerting experience. This sensation, often described as a rhythmic thumping or vibration throughout the chest, can be attributed to several factors, particularly when you have a history of mild mitral valve prolapse and have undergone thorough cardiac evaluations, including echocardiograms and Holter monitoring, which returned normal results.
Firstly, it’s important to understand that the heart's rhythm can be influenced by various physiological and psychological factors. In your case, the sensation of your heartbeat may be exacerbated by the position you are in (lying down), which can amplify the perception of heartbeats due to the proximity of the heart to the chest wall. When you are in a supine position, the heart is closer to the surface of the chest, making the beats feel more pronounced.
Mitral valve prolapse (MVP) is a common condition where the valve between the heart's left atrium and left ventricle doesn't close properly. While MVP is often benign, it can sometimes lead to palpitations or a sensation of a racing heart, particularly in situations of stress or anxiety. The fact that you have experienced these sensations during rest, especially when trying to sleep, may indicate a heightened awareness of your heartbeat, which can be linked to anxiety or stress. Anxiety can lead to increased sympathetic nervous system activity, which can heighten your perception of bodily sensations, including your heartbeat.
Additionally, factors such as caffeine intake, dehydration, or even certain medications can contribute to an increased awareness of your heartbeat. Caffeine, in particular, is a stimulant that can increase heart rate and make you more sensitive to your heart's rhythm. If you consume caffeinated beverages, it might be worth reducing your intake to see if it alleviates the sensations.
Another aspect to consider is the role of the autonomic nervous system, which regulates involuntary bodily functions, including heart rate. Dysregulation of this system can lead to symptoms such as palpitations or the sensation of your heart racing. Stress and anxiety can trigger this dysregulation, leading to a cycle of heightened awareness and anxiety about your heart's function.
Given that your cardiac evaluations have returned normal results, it is unlikely that there is a serious underlying cardiac issue. However, if these sensations persist or worsen, it would be prudent to consult with a healthcare provider, possibly a cardiologist or a neurologist, to explore further evaluations or management strategies. They may recommend lifestyle modifications, stress management techniques, or even cognitive behavioral therapy if anxiety is a contributing factor.
In summary, the sensation of feeling your heartbeat, especially when lying down, can be attributed to a combination of physiological factors related to your heart's structure and function, as well as psychological factors such as anxiety. Maintaining a healthy lifestyle, managing stress, and possibly reducing caffeine intake may help alleviate these sensations. If symptoms persist, further evaluation may be warranted to ensure comprehensive care.
