Hemorrhoidal itching
Hello Doctor: About a month ago, after returning from Japan, I experienced constipation.
After a hard bowel movement, I developed an external hemorrhoid, which was initially very uncomfortable and painful.
The doctor prescribed me a hemorrhoid ointment, which I have been applying for about a month.
During this month, I have been drinking a lot of water, and my bowel movements have become softer.
The external hemorrhoid has shrunk and feels like it is almost gone! However, recently I have been experiencing occasional itching around the anus, and the skin above the external hemorrhoid at the anal opening has become wrinkled.
What could be the reason for this?

Dr. Cai Xingzhi reply Surgery
It is likely an internal hemorrhoid (not an external hemorrhoid).
Please maintain proper hygiene after bowel movements according to hemorrhoid prevention and treatment methods.
Since intestinal fluid may leak from the anus, causing itching, simply wash the anal area with soap after bowel movements.

More Info
Itchy hemorrhoids can be a frustrating and uncomfortable condition, often exacerbated by factors such as constipation, straining during bowel movements, and inadequate hygiene. In your case, it seems that you have been dealing with external hemorrhoids, which can cause discomfort, pain, and itching, especially after a bout of constipation.
Causes of Itchy Hemorrhoids
1. Irritation and Inflammation: Hemorrhoids are swollen veins in the rectal area, and when they become inflamed, they can lead to itching. The skin around the anus is sensitive, and any irritation can trigger an itchy sensation.
2. Hygiene Issues: Inadequate cleaning after bowel movements can lead to irritation and itching. Conversely, over-washing or using harsh soaps can also irritate the skin.
3. Dietary Factors: A diet low in fiber can lead to constipation, which in turn can exacerbate hemorrhoids. While you mentioned that you have been drinking more water and your bowel movements have become softer, dietary choices still play a crucial role. Foods that are spicy or acidic can also irritate the anal area.
4. Skin Conditions: Conditions such as eczema or fungal infections can also cause itching around the anus. If the skin appears wrinkled or irritated, it may be worth considering other dermatological issues.
5. Hemorrhoid Treatment: The medication you have been using may help reduce the size of the hemorrhoids, but it might not address the underlying irritation or inflammation completely. Some topical treatments can also cause skin irritation if used excessively.
Solutions and Recommendations
1. Maintain Good Hygiene: Ensure that you clean the anal area gently after each bowel movement. Using moist wipes or a bidet can help reduce irritation compared to dry toilet paper.
2. Avoid Irritants: Steer clear of scented soaps, lotions, or any products that might irritate the sensitive skin around the anus. Opt for hypoallergenic products instead.
3. Dietary Adjustments: Continue to consume a high-fiber diet, which includes fruits, vegetables, and whole grains. This will help prevent constipation and reduce the strain during bowel movements. Additionally, consider avoiding spicy foods, caffeine, and alcohol, which can exacerbate symptoms.
4. Topical Treatments: If the itching persists, you may want to consult your doctor about using a topical corticosteroid cream to reduce inflammation and itching. However, these should be used sparingly and under medical supervision to avoid skin thinning.
5. Warm Baths: Soaking in a warm bath can provide relief from itching and discomfort. This can help soothe the area and promote healing.
6. Consult a Specialist: If the itching continues or worsens, it may be beneficial to see a healthcare provider, such as a gastroenterologist or a proctologist. They can assess your condition more thoroughly and rule out other potential issues.
7. Monitor Symptoms: Keep track of your symptoms, including when the itching occurs and any other associated symptoms. This information can be helpful for your healthcare provider in determining the best course of action.
In conclusion, while itchy hemorrhoids can be a common issue, addressing the underlying causes and maintaining proper care can significantly alleviate discomfort. If symptoms persist or worsen, seeking professional medical advice is essential to ensure that there are no other underlying conditions contributing to your symptoms.
