Postoperative scar tissue issues after hemorrhoid surgery?

Hello, Doctor :) I underwent surgery on March 5th of this year to remove a grade IV internal hemorrhoid and necrotic tissue around the anus.
I had the procedure done at a cosmetic clinic that specializes in minimally invasive surgery, and I didn't need to be hospitalized.
It has been over a month since the surgery, and the wound should have healed by now.
I haven't had much trouble with bowel movements, although I occasionally experience slight bleeding from anal fissures.
However, I have a very large scar tissue, approximately one centimeter in size, which is more severe than before the surgery.
I am quite concerned about the aesthetic aspect and really want a nice-looking anus.
I have read online that scars will gradually flatten over time, but mine is exceptionally large and not just a small skin tag.
Is there any method to address this and help me achieve a smooth and beautiful appearance? Thank you!

After undergoing hemorrhoid surgery, it's common for patients to have concerns about the appearance and healing of their surgical scars. In your case, it seems that you are experiencing significant scarring at the surgical site, which is understandably distressing, especially given your desire for a smooth and aesthetically pleasing outcome.

Firstly, it's important to recognize that healing after surgery can vary greatly from person to person. Factors such as individual skin type, the extent of the surgery, and how well you follow post-operative care instructions can all influence the healing process. In general, scars can take several months to mature and may initially appear raised, red, or discolored before gradually flattening and fading over time.

Here are some strategies that may help improve the appearance of your scar and promote a smoother recovery:
1. Scar Management Techniques:
- Silicone Gel Sheets or Silicone Gel: These products are often recommended for scar management. They can help flatten and soften scars over time. Applying silicone gel sheets or silicone gel to the scar can be beneficial, and they are typically used for several hours a day for a few months.

- Massage: Gentle massage of the scar tissue can help break down collagen fibers and improve blood flow to the area. Use a moisturizing lotion or oil to facilitate the massage, and do this for about 5-10 minutes a few times a day.

2. Topical Treatments:
- Hydrating Creams: Keeping the scar moisturized can help improve its appearance. Look for creams that contain ingredients like vitamin E, aloe vera, or hyaluronic acid.

- Prescription Creams: In some cases, dermatologists may prescribe topical treatments that can help with scar appearance, such as corticosteroid creams or other topical agents.

3. Avoid Sun Exposure: Protecting the scar from sun exposure is crucial, as UV rays can darken scars and make them more noticeable. Use sunscreen on the scar when exposed to sunlight, or cover it with clothing.

4. Consult a Dermatologist: If the scar remains prominent or if you are concerned about its appearance, consider consulting a dermatologist. They can evaluate the scar and may recommend treatments such as laser therapy, chemical peels, or microdermabrasion, which can help improve the texture and color of the scar.

5. Patience and Time: Remember that scar maturation can take up to a year or more. Many scars will continue to improve in appearance over time, so it is essential to be patient and consistent with your scar management routine.

6. Healthy Lifestyle: Maintaining a healthy diet, staying hydrated, and avoiding smoking can also support your skin's healing process. Nutrients such as vitamin C, zinc, and protein are vital for skin repair.

In summary, while it is understandable to be concerned about the appearance of your scar after hemorrhoid surgery, there are several methods you can employ to improve its look. Consistent care, patience, and possibly professional guidance from a dermatologist can help you achieve a smoother and more aesthetically pleasing outcome. If you have ongoing concerns or if the scar does not improve, do not hesitate to reach out to your healthcare provider for further evaluation and personalized recommendations.
