Menstrual irregularities in adolescents?

I had my first menstrual period, and after eight months, I got my second one.
However, after four months of regular cycles, it stopped again, which seems to have happened when I was in sixth grade.
I am now in the ninth grade, 152 cm tall and weigh 69 kg.
Could this indicate a problem with polycystic ovary syndrome (PCOS) or another issue? If I exercise and lose weight, will my menstrual cycle return to normal?

Xiao Yu, 10~19 year old female. Ask Date: 2016/05/04

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, based on your description, it is common for females under the age of 18 to have immature ovarian function, resulting in irregular menstrual cycles.
It is recommended to consult a gynecologist for relevant examinations to address any further questions.
Thank you! Wishing you peace and health.

Irregular menstruation in teenagers can be a common concern, and understanding its causes and potential solutions is essential for managing this issue effectively. In your case, experiencing a delay in your menstrual cycle for several months, followed by a return to normalcy and then another cessation, raises valid questions about your reproductive health.

One of the potential causes of irregular menstruation in adolescents is hormonal imbalance. During the teenage years, the body undergoes significant hormonal changes as it transitions from childhood to adulthood. The hypothalamus, pituitary gland, and ovaries work together to regulate the menstrual cycle, and any disruption in this system can lead to irregular periods. Conditions such as Polycystic Ovary Syndrome (PCOS) are often associated with irregular menstruation, especially if accompanied by symptoms like weight gain, acne, or excessive hair growth. However, it’s important to note that not all irregular cycles indicate a serious condition like PCOS.

Your weight and height suggest a Body Mass Index (BMI) that falls into the overweight category. Excess weight can contribute to hormonal imbalances, which may exacerbate menstrual irregularities. Engaging in regular physical activity and adopting a balanced diet can help manage your weight and may positively influence your menstrual cycle. Exercise is known to help regulate hormones and improve overall health, which can lead to more regular periods. However, it’s crucial to approach weight loss in a healthy manner, focusing on sustainable lifestyle changes rather than extreme dieting or excessive exercise.

In addition to weight management, other factors can influence menstrual regularity, including stress, sleep patterns, and overall health. High levels of stress can disrupt the hormonal balance necessary for a regular cycle, while poor sleep can also affect hormonal regulation. Ensuring you have a supportive environment, adequate sleep, and effective stress management techniques can be beneficial.

If you are concerned about the possibility of PCOS or other underlying health issues, it would be wise to consult with a healthcare provider. They can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly an ultrasound to assess the ovaries. This will help determine if there are any underlying conditions that need to be addressed.

In summary, irregular menstruation in teenagers can stem from various factors, including hormonal imbalances, weight issues, stress, and lifestyle choices. Engaging in regular exercise, maintaining a healthy diet, and managing stress can help restore menstrual regularity. However, if irregularities persist or are accompanied by other concerning symptoms, seeking medical advice is crucial for proper diagnosis and treatment. Remember, every individual is different, and what works for one person may not work for another, so personalized care is essential.
