Irregular menstruation

Hello doctor, I am 20 years old.
I got my first period when I was in the first year of junior high school, but it was irregular at that time (there were periods where I didn't have it for almost six months).
I went to see a gynecologist, had an ultrasound, and the doctor prescribed medication to regulate my menstrual cycle.
After that, it became more regular (but the timing was inconsistent: the first month it would come at the beginning, the next month in the middle, and then at the end of the month).
However, during my senior year of high school, it became irregular again.
I visited a gynecologist to confirm that there were no abnormalities in my uterus, and I was prescribed medication again.
Later, I tried traditional Chinese medicine to improve my overall health, and after that, my periods became more regular, so I stopped taking the medication.
Currently, in my third year of college, I frequently experience irregular periods or have long gaps between them (up to 42 days).
I have seen gynecologists who have confirmed that there are no abnormalities, and they have prescribed medication.
Is there any way to improve the issue of irregular menstruation? Thank you.

Hello, Miss Apple: Common causes of irregular menstrual cycles include emotional factors, stress, irregular sleep patterns, frequent late nights, inconsistent meal times, being overweight or underweight, and prolonged intense exercise and training.
It is advisable to make adjustments in these areas.
If there is still no improvement, you may consider using medication to help regulate your menstrual cycle.

Irregular menstruation is a common concern among young women, and it can be influenced by various factors including hormonal imbalances, stress, lifestyle changes, and underlying health conditions. As a 20-year-old woman who has experienced irregular periods since your first menstruation, it's understandable that you are seeking effective solutions to manage this issue.

Firstly, it’s important to recognize that menstrual cycles can vary significantly from person to person. A typical menstrual cycle ranges from 21 to 35 days, and cycles can be irregular, especially during the first few years after menarche (the first menstrual period). However, if your cycles are consistently longer than 35 days or if you experience significant gaps between periods, it may warrant further investigation.

Here are some effective solutions and strategies to help manage irregular menstruation:
1. Consultation with a Healthcare Provider: Since you have already seen gynecologists and undergone ultrasounds, it’s essential to continue working closely with your healthcare provider. They can help rule out any underlying conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or other hormonal imbalances that could be affecting your menstrual cycle.

2. Hormonal Treatments: If your irregular cycles are due to hormonal imbalances, your doctor may recommend hormonal contraceptives such as birth control pills. These can help regulate your menstrual cycle by providing consistent hormone levels. It’s important to discuss the benefits and potential side effects with your doctor.

3. Lifestyle Modifications: Lifestyle factors play a significant role in menstrual health. Regular exercise, a balanced diet, and maintaining a healthy weight can positively impact your menstrual cycle. Stress management techniques such as yoga, meditation, or mindfulness can also help regulate hormones and improve overall well-being.

4. Nutritional Support: Certain vitamins and minerals, such as vitamin D, magnesium, and omega-3 fatty acids, can support hormonal balance. A diet rich in whole foods, including fruits, vegetables, whole grains, and lean proteins, can provide the nutrients your body needs to function optimally.

5. Alternative Therapies: Since you mentioned trying traditional Chinese medicine (TCM), it’s worth noting that acupuncture and herbal remedies may help some women regulate their cycles. However, it’s crucial to work with a qualified practitioner who understands your specific health needs.

6. Monitoring Your Cycle: Keeping a menstrual diary can help you track your cycles, symptoms, and any factors that may influence your menstruation. This information can be valuable for your healthcare provider in determining the best course of action.

7. Consideration of Underlying Conditions: If irregular periods persist, further evaluation may be necessary. Conditions such as PCOS, endometriosis, or thyroid disorders can cause irregular menstruation and may require specific treatments.

In summary, managing irregular menstruation involves a combination of medical evaluation, lifestyle adjustments, and possibly hormonal treatments. It’s essential to maintain open communication with your healthcare provider to find the most suitable approach for your individual situation. Remember that while irregular periods can be frustrating, many young women experience similar challenges, and with the right support and strategies, it is possible to achieve a more regular menstrual cycle.
