Painful Red Spot Under Toenail: Causes and Solutions - Dermatology

The flesh under the nail is very painful?

There is a small red spot under the skin of my left big toe, which causes significant pain in the entire toe (the red spot is located on the left side of the toe).
It hurts even when I'm not moving it, and pressing on it makes the pain worse.
Walking also causes pain.
I haven't injured or bumped it recently, so I'm not sure what the issue is.

Painful red spots under the toenail can be concerning and may arise from various causes. Given your description of a painful red spot under your left big toe, it is essential to consider several potential issues that could be at play.

1. Subungual Hematoma: One of the most common causes of a painful red spot under the toenail is a subungual hematoma, which occurs when blood collects between the nail bed and the nail itself. This can happen due to trauma, even if you do not recall any specific injury. The pressure from the accumulated blood can cause significant pain, and the area may appear red or dark. If the hematoma is large, it may require drainage to relieve the pressure.

2. Ingrown Toenail: Another possibility is an ingrown toenail, where the edge of the toenail grows into the surrounding skin. This can lead to redness, swelling, and pain, especially if there is an infection. The area may become tender to touch, and you might notice pus or increased warmth around the nail.

3. Fungal Infection: Fungal infections can also cause discoloration and pain under the nail. These infections often lead to thickening of the nail, discoloration, and sometimes a foul odor. If you notice any changes in the texture or color of the nail, this could be a contributing factor.

4. Bacterial Infection: If there is an open wound or a break in the skin around the toenail, bacteria can enter and cause an infection. This may present as redness, swelling, and increased pain. In severe cases, you might also see pus or experience fever.

5. Nail Disorders: Various nail disorders, such as psoriasis or other skin conditions, can also manifest as painful spots under the nail. These conditions may require specific treatments and can be chronic.

6. Other Causes: Less commonly, systemic conditions such as vasculitis or other inflammatory diseases can lead to changes in the nails and surrounding tissues. If you have other symptoms, such as joint pain or skin changes, it may be worth discussing these with a healthcare provider.

Solutions and Recommendations:
- Observation: If the pain is mild and there are no signs of infection (such as pus or fever), you might consider monitoring the situation for a few days. Sometimes, minor issues resolve on their own.

- Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage pain and reduce inflammation.

- Warm Soaks: Soaking your foot in warm, soapy water can help alleviate discomfort and keep the area clean.

- Avoid Pressure: Try to avoid tight shoes and pressure on the affected toe. Wearing open-toed shoes or sandals may provide relief.

- Seek Medical Attention: If the pain worsens, if you notice increased redness, swelling, or discharge, or if you develop a fever, it is crucial to seek medical attention. A healthcare provider can assess the situation, potentially drain a hematoma, treat an infection, or address any underlying conditions.

- Preventive Measures: Once the issue is resolved, consider preventive measures such as keeping your toenails trimmed straight across, wearing appropriate footwear, and maintaining foot hygiene to prevent future occurrences.

In summary, while a painful red spot under the toenail can arise from various causes, it is essential to monitor the symptoms closely and seek medical attention if necessary. Early intervention can prevent complications and ensure proper treatment.
