Gynecomastia

I have been overweight since the third grade of elementary school, and I still am.
Currently, I weigh around 84 kg and my height is approximately 173 cm.
When viewed from the side, my chest appears protruding.
I have learned a bit about gynecomastia in males.
My chest feels soft when pressed down.
However, I am unsure about the difference in softness between fat and glandular tissue.
The area around my nipples also feels normal; the areola does not exceed 2 cm when pinched, and it's less than 1 cm.
I am curious if I have gynecomastia.

Sesshomaru, 10~19 year old female. Ask Date: 2016/05/11

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear Sesshomaru: Sometimes it is due to adipose hypertrophy, but it can also be accompanied by hyperplasia of the breast tissue.
In overweight males, due to the influence of female hormones, there is often varying degrees of glandular hyperplasia.
Observation is usually sufficient, but if there is a persistent increase in size, a clinic evaluation should be conducted.

More Info

Gynecomastia, commonly referred to as "man boobs," is a condition characterized by the enlargement of breast tissue in males. It can be a source of concern for many men, particularly during adolescence when hormonal changes are prevalent. Understanding the signs, symptoms, and potential concerns associated with gynecomastia is essential for those who may be experiencing this condition.

Signs and Symptoms of Gynecomastia
1. Breast Enlargement: The most noticeable sign of gynecomastia is the enlargement of breast tissue. This can occur in one or both breasts and may present as a firm or soft mass beneath the nipple.

2. Softness of Tissue: In your case, you mentioned that your chest feels soft when pressed. This is typical in gynecomastia, where the breast tissue is primarily glandular rather than fatty. It's important to differentiate between gynecomastia and pseudogynecomastia, which is fat accumulation in the breast area without glandular tissue enlargement.

3. Nipple Sensitivity: Some men with gynecomastia may experience tenderness or sensitivity around the nipple area. This can be due to hormonal changes affecting the breast tissue.

4. Changes in Nipple Appearance: In some cases, the nipples may appear more prominent or may have a different texture compared to the surrounding skin.

Causes of Gynecomastia
Gynecomastia can result from various factors, including:
- Hormonal Imbalances: An imbalance between testosterone and estrogen levels is the primary cause of gynecomastia. Increased estrogen levels or decreased testosterone levels can lead to breast tissue growth.

- Medications: Certain medications, including anti-androgens, anabolic steroids, and some antidepressants, can contribute to the development of gynecomastia.

- Health Conditions: Conditions such as liver disease, kidney disease, hyperthyroidism, and tumors affecting the testes or adrenal glands can also lead to hormonal imbalances that result in gynecomastia.

- Obesity: Excess body fat can lead to increased estrogen production, which may contribute to breast tissue enlargement.

Concerns and Considerations
1. Psychological Impact: Many men with gynecomastia experience psychological distress, including embarrassment, anxiety, and low self-esteem. This can affect social interactions and overall quality of life.

2. Underlying Health Issues: While gynecomastia itself is usually benign, it can sometimes indicate underlying health problems. If you notice sudden changes in breast size or if it is accompanied by other symptoms, it is essential to consult a healthcare provider.

3. Diagnosis: A healthcare provider will typically perform a physical examination and may order blood tests to check hormone levels. Imaging studies, such as ultrasound or mammography, may be used to differentiate between gynecomastia and other breast conditions.

4. Treatment Options: Treatment for gynecomastia may not be necessary if the condition is mild and not causing significant discomfort. However, if it is severe or persistent, options include medication to address hormonal imbalances or surgical procedures to remove excess breast tissue.

Conclusion
Based on your description, it seems you may be experiencing gynecomastia, especially if the breast tissue is soft and there is no significant fat accumulation. It is advisable to consult with a healthcare provider who can evaluate your symptoms, perform necessary tests, and discuss potential treatment options if needed. Understanding the condition and its implications can help you make informed decisions about your health and well-being.
