Small bumps under the foreskin follicles?
There are small bumps slightly larger than pimples in the follicular area of the foreskin.
Sometimes, a white, pasty substance can be squeezed out, distributed from the middle to the base of the penis.
In the worst case, it appears inflamed, red, and swollen, and it hurts when pressed.
I wash it thoroughly during showers and try to wear breathable underwear, but there has been no improvement.
What could be the cause? Should I see a dermatologist or a urologist?

Dr. Li Xinyi reply Urology
It could be that some follicular cysts become inflamed and red due to being protruded, which may happen if the skin is broken or if they are squeezed, causing a wound.
Doctors generally do not treat them because they are benign.
If there are concerns, it is advisable to see a dermatologist or urologist for an examination and to discuss with the physician.

More Info
Understanding small bumps under the foreskin can be concerning, especially when they are associated with discomfort, inflammation, or discharge. Based on your description, these bumps could be sebaceous cysts, folliculitis, or even pearly penile papules, which are common and usually harmless. However, the presence of pain, redness, and the ability to express a white substance suggests a potential infection or inflammation.
Possible Causes:
1. Sebaceous Cysts: These are small lumps that can occur when sebaceous glands become blocked. They can sometimes become inflamed or infected, leading to pain and discharge.
2. Folliculitis: This is an infection of the hair follicles, often caused by bacteria. It can lead to red, inflamed bumps that may be painful and can sometimes produce pus.
3. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans. They are not a sign of infection and do not require treatment.
4. Infections: Conditions such as sexually transmitted infections (STIs) or other bacterial infections can also present with similar symptoms. If the bumps are painful, red, and produce discharge, it may indicate an infection that requires medical attention.
Treatment Options:
1. Good Hygiene: Continue to maintain good hygiene by washing the area gently with mild soap and water. Avoid harsh soaps or scrubs that can irritate the skin.
2. Warm Compresses: Applying a warm compress to the affected area can help reduce inflammation and promote drainage if there is an infection.
3. Topical Antibiotics: If there is an infection, topical antibiotics may be helpful. However, these should be used under the guidance of a healthcare provider.
4. Avoid Squeezing: While it may be tempting to squeeze the bumps, this can lead to further irritation or infection. It’s best to leave them alone unless advised otherwise by a healthcare professional.
5. Consult a Healthcare Provider: If the bumps persist, worsen, or are accompanied by other symptoms (such as fever, increased pain, or systemic symptoms), it is important to seek medical attention.
Which Specialist to See:
Given the location of the bumps, you may consider visiting either a dermatologist or a urologist. A dermatologist specializes in skin conditions and can assess the bumps for any dermatological issues, while a urologist specializes in urinary tract and male reproductive health and can evaluate for any potential infections or conditions related to the genital area.
When to Seek Immediate Care:
- If you experience severe pain, fever, or swelling.
- If the bumps begin to drain pus or blood.
- If you notice any changes in your overall health or new symptoms.
In summary, while small bumps under the foreskin can be common and often benign, the presence of pain and inflammation warrants a thorough evaluation. Maintaining good hygiene and consulting with a healthcare provider will help ensure proper diagnosis and treatment.
