Excuse me, pharmacist?
Hello, pharmacist.
I apologize for not finishing my message last time and accidentally sending it.
The main point is that I want to ask about the insomnia medication I take.
In the morning, I take a 0.5 mg capsule of Mecobalamin (Methylcobalamin) and a 0.25 mg tablet of Sertraline.
Before bed, I take a 0.5 mg tablet of Clonazepam, which I just switched to on Monday, along with the 0.25 mg Sertraline, 500 mg of Calcium Carbonate, and a self-paid dietary supplement called Xinpining, which costs 25 TWD per tablet.
I originally needed 56 tablets but now I have an additional 28 tablets.
I mentioned to the doctor during my follow-up appointment yesterday that I had stopped taking the Sertraline.
The doctor seemed concerned and emphasized that I must continue taking the 0.25 mg Sertraline, stating that this medication is important for my stomach and intestines.
However, I do not have gastrointestinal issues, and I have been taking it for almost four years.
For the first two years, I took it regularly, but over time, I started experiencing blurred vision, memory decline, and muscle twitching.
I reported these symptoms to the doctor, but he insisted that they wouldn't be caused by the medication.
I am hesitant to continue taking it, but I am feeling somewhat better now.
Could you please explain to me why the doctor insists that I must take the 0.25 mg Sertraline?

Dear Patient,
Thank you for reaching out with your concerns regarding your medication regimen. It’s essential to understand the importance of each medication you are prescribed, especially when it comes to managing conditions like insomnia and gastrointestinal health. Let’s break down the medications you mentioned and their significance.
1. Understanding Your Medications:
- 生達癒利舒盼錠 (Sodium Valproate): This medication is primarily used to treat epilepsy and bipolar disorder, but it can also have a role in managing mood disorders and certain types of anxiety. It is crucial for stabilizing mood and preventing seizures. Your doctor may emphasize its importance due to its role in maintaining neurological stability, which can be particularly important if you have a history of mood disorders or seizures.
- 碳酸鈣錠 (Calcium Carbonate): This is often used as a dietary supplement to ensure adequate calcium intake, which is vital for bone health. It can also act as an antacid to relieve symptoms of indigestion or heartburn. If you have been on long-term medications that can affect your bone density or gastrointestinal health, calcium supplementation may be necessary.
- 欣波寧 (Nutritional Supplement): While not a medication per se, nutritional supplements can play a role in overall health, especially if you have specific deficiencies or dietary restrictions. However, it’s essential to discuss the necessity and efficacy of any supplement with your healthcare provider.
2. Concerns About Side Effects:
- You mentioned experiencing side effects such as blurred vision, memory issues, and muscle twitching. These symptoms can be concerning and may warrant further investigation. While some side effects can be associated with long-term use of certain medications, it’s crucial to differentiate between medication side effects and symptoms of underlying conditions.
- It’s important to communicate these side effects to your healthcare provider. They may consider adjusting your dosage or switching to an alternative medication that may have a more favorable side effect profile for you.
3. Why Your Doctor Emphasizes Certain Medications:
- Your doctor’s insistence on continuing the 生達癒利舒盼錠 may stem from its importance in your overall treatment plan. If it has been effective in managing your symptoms, the benefits may outweigh the risks of side effects.
- Additionally, some medications require a gradual tapering off rather than an abrupt stop to avoid withdrawal symptoms or a resurgence of the condition being treated.
4. Managing Your Health:
- If you feel that your current regimen is causing more harm than good, it’s crucial to have an open dialogue with your healthcare provider. They can help you weigh the risks and benefits of each medication and explore alternatives if necessary.
- Lifestyle modifications, such as improving sleep hygiene, reducing caffeine intake, and engaging in regular physical activity, can also complement your treatment plan and help manage insomnia and anxiety.
5. Follow-Up:
- Regular follow-ups with your healthcare provider are essential to monitor your progress and adjust your treatment plan as needed. If you have concerns about the necessity of a medication, don’t hesitate to ask for a second opinion or discuss your feelings with your doctor.
In conclusion, understanding the role of each medication in your treatment plan is vital for your health and well-being. Always communicate openly with your healthcare provider about any side effects or concerns you may have. Your health is a partnership between you and your healthcare team, and your input is invaluable in managing your treatment effectively.
Wishing you good health and peace of mind.
