Can braces help adjust a "braces face" back to normal?
Hello, I have been undergoing orthodontic treatment for three years, and I have developed a noticeable "braces face" (sunken temples, protruding cheekbones, and hollow cheeks).
During the treatment, I gained ten kilograms, and my round face has become more angular.
I discussed with my doctor the possibility of using a headgear to push back my cheekbones, but the doctor said that my front teeth gaps have not fully closed, making headgear unsuitable.
The doctor mentioned that they cannot address this issue and suggested hyaluronic acid injections or consulting another doctor for potential solutions.
Can widening the dental arch improve the appearance of braces face? Are there any other recommended methods? I really do not want to resort to cosmetic procedures.
Is there any other way to reposition or pull back my cheekbones? The doctor stated that orthodontics is unrelated to the protrusion of the cheekbones, but I used to have a round face, and now my temples are noticeably sunken, and my cheekbones are indeed protruding.
I am not sure if this is related, but I read online that about 70% of people experience this issue, which is very concerning for me.
I initially sought orthodontic treatment for aesthetic reasons, but now I feel like I am facing depression due to this situation.

Hello, regarding the repositioning of the zygomatic bone, this will not be affected during orthodontic treatment.
The position of the zygomatic bone does not change; rather, it is the alignment of the teeth in the oral cavity that changes, leading to alterations in the occlusion pattern, which in turn affects the appearance of the skin.
This is why you may have noticed the so-called "braces face" before and after treatment.
You are still in the orthodontic phase, and I recommend that you continue to complete your treatment.
Orthodontics aims to establish a proper occlusion, which will allow the soft tissues around the eyelids to adapt to these changes.
For the areas around the eyelids that appear sunken, since there is less fat in that region, you may consider consuming some gelatinous foods (in moderation) during this period, and discuss this with your doctor during follow-up visits.
Please do not worry too much.
Wishing you peace and health.
Sincerely, Dr.

Orthodontics can indeed have an impact on facial structure, but the extent and nature of these changes can vary significantly from person to person. In your case, it seems that you are experiencing what is commonly referred to as "braces face," which can manifest as changes in facial contour, such as hollowed cheeks or pronounced cheekbones. This phenomenon can be particularly distressing, especially when the initial intention of orthodontic treatment was to enhance aesthetics.
Firstly, it is important to understand that orthodontic treatment primarily focuses on the alignment of teeth and the correction of bite issues. While these changes can indirectly affect facial appearance—due to the repositioning of teeth and changes in jaw alignment—the actual skeletal structure of the face, including the cheekbones and temples, is not typically altered by braces alone. The changes you are observing, such as the hollowed temples and pronounced cheekbones, may be influenced by factors such as weight gain or loss, muscle tone, and even the natural aging process during the time of your treatment.
Regarding your specific concerns about the appearance of your face post-treatment, there are a few avenues you might consider:
1. Weight Management: Since you mentioned gaining weight during your orthodontic treatment, focusing on a balanced diet and regular exercise could help improve your overall facial appearance. Sometimes, weight fluctuations can significantly affect how the face looks, and losing excess weight may help restore some of the fullness to your cheeks.
2. Facial Exercises: Engaging in facial exercises may help tone the muscles around your cheeks and jawline, potentially improving the overall contour of your face. While scientific evidence on the effectiveness of facial exercises is limited, some individuals report positive results.
3. Consultation with a Specialist: If you are concerned about the prominence of your cheekbones or the hollowness of your temples, consulting with a facial plastic surgeon or a dermatologist specializing in aesthetic treatments could provide you with options that do not involve invasive procedures. They may suggest non-surgical options like fillers, which can add volume to specific areas of the face without the need for surgery.
4. Orthodontic Adjustments: If you are still in the process of orthodontic treatment, discussing your concerns with your orthodontist may yield some solutions. They might be able to adjust your treatment plan to address your concerns about facial aesthetics, although it is essential to understand that the primary goal remains the alignment of your teeth and bite.
5. Psychological Support: It is also crucial to address the emotional impact that these changes have had on you. If you are feeling distressed or experiencing symptoms of depression, seeking support from a mental health professional can be beneficial. They can help you navigate these feelings and provide coping strategies.
In summary, while orthodontics can influence facial structure, the changes you are experiencing may not be directly reversible through orthodontic treatment alone. Exploring options such as weight management, facial exercises, and consultations with specialists can help you address your concerns. Remember that your self-image is important, and seeking support—both physically and emotionally—can lead to a more positive outlook on your appearance.
