Postoperative Issues After Right Shoulder Arthroscopy

Is it normal to experience numbness, swelling, and weakness in the right calf and foot two days after right shoulder surgery (with no swelling in the outer area)? It feels very stiff, and there is no sensation when touched.
Thank you for your response.

Dr. Hong Ximing reply Orthopedics

Numbness and swelling in the calf after shoulder surgery should not be related to the surgery itself.
There may be other causes, and it is advisable to consult with the attending physician for further evaluation.

After right shoulder surgery, experiencing numbness and swelling in the right leg, particularly from the calf to the foot, can be concerning. While some degree of numbness and swelling can occur post-operatively due to various factors such as anesthesia, positioning during surgery, or even nerve irritation, the symptoms you describe warrant careful consideration.

Firstly, it is essential to understand that during surgery, especially if it involves the shoulder, the positioning of the body can lead to temporary nerve compression or irritation. If your arm was positioned in a way that put pressure on the nerves that run down to your leg, this could explain the numbness and weakness you are feeling in your right leg. However, this should typically resolve within a few days as the pressure is relieved.

The swelling you are experiencing could also be attributed to a few factors. Post-surgical swelling is common and can occur due to fluid accumulation in the tissues as a response to the surgical trauma. However, if the swelling is significant and accompanied by numbness, it could indicate a more serious issue, such as a blood clot or deep vein thrombosis (DVT), especially if you have limited mobility after surgery.

It is also important to consider the possibility of a nerve injury. While rare, surgical procedures can sometimes inadvertently affect nearby nerves, leading to symptoms like numbness, tingling, or weakness in the extremities. If the numbness persists or worsens, or if you experience additional symptoms such as severe pain, changes in skin color, or increased swelling, it is crucial to seek medical attention promptly.

In terms of management, here are a few recommendations:
1. Monitor Symptoms: Keep track of your symptoms, noting any changes in numbness, swelling, or pain. If you notice any worsening or new symptoms, contact your healthcare provider.

2. Rest and Elevation: Resting the affected leg and elevating it can help reduce swelling. Avoid putting weight on the leg until you have been cleared by your doctor.

3. Gentle Movement: If advised by your healthcare provider, gentle movements or exercises may help improve circulation and reduce stiffness. However, avoid any activities that cause pain or discomfort.

4. Pain Management: Over-the-counter pain relievers may help manage discomfort, but consult your doctor before taking any medication, especially if you are on other medications post-surgery.

5. Follow-Up Appointment: Ensure you attend any scheduled follow-up appointments with your surgeon or healthcare provider. They will assess your recovery and address any concerns you may have.

In conclusion, while some numbness and swelling can be normal after surgery, the specific symptoms you describe—especially the numbness extending to your leg—should be evaluated by your healthcare provider. It is always better to err on the side of caution when it comes to post-operative symptoms, as early intervention can prevent complications and ensure a smoother recovery. If you have not already done so, please reach out to your surgeon or healthcare provider to discuss your symptoms in detail.
