Penile blunt trauma caused by exercise, resulting in curvature?

Hello, Doctor.
I am Mr.
Lin, 29 years old.
I have some questions regarding penile bruising and would like to hear your professional opinion.
At the end of December last year, while playing badminton, I accidentally hit the tip of my penis with the racket.
It was painful at the moment, and after two days, I noticed a severe curvature at the junction below the glans during erections.
There was no visible bruising on the surface, and the pain was manageable, but I experienced a strong feeling of tightness, with only about 50% of the usual erectile fullness.
I consulted two urologists, and after an ultrasound, they both said there was nothing serious and recommended observation for self-healing.
They prescribed a medication called Cialis (5mg) to enhance blood circulation and clear any bruising, but after four weeks of use, there was no significant improvement, so I did not return for a follow-up.
During this time, I also tried acupuncture, which initially showed good results and significantly improved the congestion, and now, about five months later, I have recovered to about 75%.
However, the curvature still causes the tip of my penis to tilt to the right during erections (it originally tilted to the left), resulting in a slight S-shape.

Now that five months have passed and I am about to marry my current girlfriend next year, I am very concerned that my condition will affect our sexual life after marriage (we currently do not have a sexual relationship).
I would like to hear a third-party opinion to help me decide on the next steps.

Could you please clarify my doubts?
1.
Given that the congestion has gradually improved due to acupuncture but the curvature remains, does this indicate that there may be a tear in a certain area, such as the tunica albuginea?
2.
Is it common for curvature to heal on its own? I am now entering the sixth month; is this duration typical?
3.
Can the curvature be adjusted or treated through surgery?
4.
What further examinations can I undergo to determine if my condition can fully heal on its own?
5.
Is it possible for the curvature to completely return to its previous state? Besides surgery, are there other methods such as heat therapy or massage that could help?
Thank you very much for your time and answers, Doctor.
This issue has been troubling me for a long time, and the pressure has increased due to the upcoming marriage.
I truly hope this problem can be resolved with your help.

Hello Mr.
Lin: Here are the answers to your questions:
Q1: In the case of congestion gradually improving due to acupuncture, if the curvature issue persists, does it indicate that there may be a rupture in a certain area, such as the tunica albuginea?
A: Based on your description, it is still difficult to speculate on the current injury site.
If there is a rupture in the tunica albuginea, it often leads to erectile dysfunction.
Since the injury has lasted for six months, the likelihood of fibrosis (scarring) in the tunica albuginea is higher.
Once fibrosis occurs, it may result in abnormal curvature.
Q2: Can curvature generally heal on its own? I have already entered the sixth month; is this a typical duration?
A: After fibrosis of the tunica albuginea, it becomes more challenging for it to heal on its own.
There are some medications that may help, so please consult your physician.
Q3: Can the curvature be adjusted or treated through surgery?
A: Generally, if the curvature exceeds 30 degrees, it may affect sexual intercourse, and at that point, surgical intervention is recommended.
Q4: What further examinations can I undergo to determine if my condition can fully heal on its own?
A: If the curvature does not affect sexual intercourse, it actually does not require special treatment, as the penis is not an externally visible organ.
Q5: Can this curvature fully return to its previous state? Besides surgery, are there other methods such as heat application or massage that could help?
A: Currently, there are no therapies with guaranteed effectiveness.
It is advisable to monitor the rate of curvature progression.
If it progresses rapidly, consider medications to slow down the curvature.
Avoid trying unproven remedies to prevent worsening the condition.

Hello Mr. Lin,
Thank you for reaching out with your concerns regarding your penile bruising and the associated curvature. I understand how distressing this situation can be, especially with your upcoming marriage. Let’s address your questions one by one.

1. Regarding the curvature and potential damage to the tunica albuginea: The tunica albuginea is the fibrous envelope of the corpora cavernosa, and any trauma can potentially lead to tears or damage. The fact that you are experiencing a curvature, especially if it is pronounced, could suggest that there may be some localized scarring or fibrosis in the tunica albuginea. This could be a result of the trauma you experienced. The improvement in blood flow due to acupuncture is a positive sign, but it does not necessarily indicate that the structural integrity of the tunica has returned to normal.

2. Self-healing of the curvature: Generally, penile injuries can take time to heal, and the duration can vary significantly from person to person. While some men may experience a return to normal curvature within a few months, others may take longer. Six months is a reasonable timeframe to assess recovery, but if the curvature persists, it may not resolve completely on its own. The body can heal, but structural changes like scarring may require intervention.

3. Surgical options for curvature: Yes, surgical intervention is available for penile curvature, especially if it is causing discomfort or affecting sexual function. Procedures such as plication or grafting can be performed to correct the curvature. It is essential to consult with a urologist who specializes in penile surgery to discuss the best options for your specific situation.

4. Further examinations: To better understand the extent of any potential damage, you might consider additional imaging studies, such as a penile Doppler ultrasound, which can assess blood flow and identify any structural abnormalities. A urologist may also perform a physical examination to evaluate the curvature and any associated symptoms.

5. Possibility of complete recovery: While some men do regain their original curvature, others may not achieve complete restoration without surgical intervention. Non-surgical methods such as heat therapy or massage may provide some relief, but they are unlikely to correct significant structural issues. It is crucial to approach these methods with caution and preferably under the guidance of a healthcare professional.

In conclusion, I recommend that you schedule a follow-up appointment with a urologist who specializes in penile health. They can provide a thorough evaluation and discuss potential treatment options tailored to your needs. It’s understandable to feel anxious about this situation, especially with your upcoming marriage, but addressing these concerns proactively will help you find a resolution.

Thank you for your patience, and I hope this information helps you make informed decisions about your health. Please take care, and don’t hesitate to reach out if you have further questions.
