Penile glans contusion
Hello Doctor, I would like to ask you about a problem concerning bruising on the glans.
Two days ago, my girlfriend performed oral sex on me without a condom.
The process was fine until after ejaculation when I noticed bruising on the left side of the glans, which is elongated and approximately half the size of my pinky nail.
It doesn't hurt much, but the surface is slightly raised due to the bruising.
I have not been circumcised, and my glans is normally covered by the foreskin, which retracts during erection, so I have no plans for circumcision.
Currently, I am hesitant to puncture the bruise due to concerns about bacterial infection.
I would like to ask the doctor:
1) How should I care for this injury? If the skin breaks, what ointment should I apply? Will the bruising gradually resolve on its own?
2) What could be the cause of the bruising? Is it possible that the skin on my glans is too sensitive? (I have previously experienced slight bleeding on the right side of the glans after using a condom, which healed in a few days.)
3) We are each other's only sexual partners, and this has not happened before during oral sex.
My girlfriend was tighter and deeper during the act, could this have caused the bruising due to excessive suction? I would like to know how to avoid this in the future.
Thank you, Doctor, for taking the time to respond to my questions.

It is likely that a blood vessel under the skin was ruptured during the sexual activity, leading to a subcutaneous hemorrhage.
This is uncommon and may not be preventable.
If the epidermis is broken, you can use povidone-iodine to disinfect the area, and then visit a urologist to obtain some ointment for application.
If there is no break in the skin, generally, there will be no infection; the epidermis beneath the blood clot will regenerate, and the blood clot along with the old tissue will gradually slough off.
There is no need to worry, but it is still advisable to consult a urologist for an examination and further discussion with the physician.

When it comes to penile bruising, particularly after sexual activity such as oral sex, it's essential to understand the underlying causes, appropriate care, and preventive measures. Based on your description, it seems you have experienced a localized bruise on the left side of the glans (the head of the penis) after a sexual encounter. Here’s a comprehensive overview of how to manage this situation.
1. Understanding the Bruise and Care Tips
Bruising in the genital area can occur due to various reasons, including trauma from vigorous sexual activity. In your case, the bruise may have resulted from the pressure exerted during oral sex, especially if your partner was applying significant suction or if the movements were particularly forceful.
Care Tips:
- Rest and Avoid Irritation: Allow the area to heal by avoiding any further irritation. This means refraining from sexual activity until the bruise has healed.
- Cold Compress: Applying a cold compress to the area can help reduce swelling and discomfort. Make sure to wrap ice in a cloth to avoid direct contact with the skin.
- Observation: Monitor the bruise for changes. Bruises typically change color as they heal, transitioning from red to purple, then yellow, and finally fading away. If the bruise does not improve or worsens, seek medical attention.
- Hygiene: Maintain good hygiene to prevent any potential infections. Gently clean the area with mild soap and water.
2. What to Do If the Skin Breaks
If the bruise develops into a break in the skin, it’s crucial to keep the area clean. You can apply an over-the-counter antibiotic ointment, such as Neosporin, to prevent infection. Avoid using any harsh chemicals or alcohol-based products on the area, as these can cause irritation.
3. Causes of Bruising
The sensitivity of the skin on the glans can indeed contribute to bruising. The skin in this area is thinner and more delicate than other parts of the body, making it more susceptible to injury. If you have previously experienced minor injuries or sensitivity, it may indicate that your skin is particularly reactive to pressure or friction.
4. Preventive Measures for Future Encounters
To minimize the risk of bruising in the future, consider the following:
- Communication: Discuss with your partner about the intensity of oral sex. Let them know if you feel uncomfortable or if the pressure is too much.
- Pacing: Encourage a slower pace during oral sex, especially if you notice that certain techniques lead to discomfort or bruising.
- Lubrication: While lubrication is less common in oral sex, ensuring that there is adequate moisture can help reduce friction and potential injury.
- Positioning: Experiment with different positions that may reduce pressure on sensitive areas.
5. When to Seek Medical Attention
If you notice any of the following, it’s essential to consult a healthcare provider:
- The bruise does not improve after a week.
- There is significant pain or swelling.
- You notice any signs of infection, such as increased redness, warmth, or discharge.
- You experience any unusual symptoms, such as difficulty urinating or persistent pain.
In conclusion, while penile bruising can be concerning, it is often a result of minor trauma that can heal with proper care. By following the tips outlined above and maintaining open communication with your partner, you can minimize the risk of future occurrences. Always prioritize your health and seek medical advice if you have any doubts or concerns.
