Loss of smell after septoplasty?
Hello, doctor.
I underwent a septoplasty and uvulopalatopharyngoplasty about four weeks ago due to sleep apnea, but I have lost my sense of smell since then.
When I smell fragrances, they all have a strange odor.
How should I address this issue?
Benson, 40~49 year old female. Ask Date: 2016/06/05
Dr. Li Jiaxuan reply Otolaryngology
Olfaction is partially derived from the nasal mucosa.
Surgery for a deviated septum may temporarily reduce the surface area of the mucosa and cause transient hyposmia due to mucosal injury, which is a normal phenomenon.
It is advised to avoid exposure to overly strong odors and to have regular follow-up appointments with the surgeon to monitor the condition.
Wishing you a safe recovery.

Loss of smell, or anosmia, following septoplasty and uvulopalatopharyngoplasty (UPPP) can be a concerning issue for patients. After undergoing these surgical procedures, it is not uncommon for individuals to experience changes in their sense of smell. This can be attributed to several factors related to the surgery and the healing process.
Firstly, during septoplasty, the nasal septum is straightened, which can temporarily affect the olfactory nerves located in the nasal cavity. The olfactory nerves are responsible for transmitting smell signals from the nose to the brain. If these nerves are disturbed during surgery, it may lead to a temporary loss of smell or altered smell perception, known as parosmia, where familiar scents are perceived as strange or unpleasant.
Secondly, the healing process itself can contribute to changes in smell. Post-operative swelling, inflammation, or the presence of mucus can obstruct the olfactory receptors, leading to a diminished ability to detect odors. Additionally, if there is any nasal congestion or blockage due to swelling or scarring, this can further impair the sense of smell.
In some cases, the use of nasal packing or splints after surgery can also affect olfactory function. These materials are often used to stabilize the septum and control bleeding, but they can temporarily obstruct airflow through the nasal passages, which is essential for smell.
To address the loss of smell after septoplasty, here are some recommended steps:
1. Follow-Up Consultation: It is crucial to schedule a follow-up appointment with your ENT specialist. They can assess your nasal passages and determine if there are any obstructions or complications that need to be addressed.
2. Nasal Irrigation: Using a saline nasal spray or performing nasal irrigation can help keep the nasal passages moist and clear of mucus. This may facilitate better airflow and potentially improve your sense of smell.
3. Avoid Irritants: Stay away from strong odors, smoke, and other irritants that could exacerbate nasal inflammation or congestion. This includes avoiding strong cleaning products, perfumes, and other chemical scents.
4. Smell Training: Engaging in smell training can be beneficial. This involves regularly exposing yourself to a variety of scents (such as essential oils or spices) to help retrain your olfactory senses. This practice has shown promise in helping individuals recover their sense of smell after loss.
5. Patience: Recovery of the sense of smell can take time. It is essential to be patient as your body heals. In many cases, the sense of smell may gradually return as swelling decreases and the nasal passages clear.
6. Consider Other Factors: If your loss of smell persists, it may be worth considering other underlying factors such as allergies, sinus infections, or even neurological issues that could be contributing to the problem.
In conclusion, while loss of smell after septoplasty can be distressing, it is often a temporary condition related to the surgical procedure and the healing process. By following up with your healthcare provider and taking proactive steps to care for your nasal passages, you can work towards regaining your sense of smell. If symptoms persist or worsen, further evaluation may be necessary to rule out other potential causes.
