Pregnancy with genital warts?

I am currently 13 weeks pregnant and have been diagnosed with genital warts.
Can treatment be done during this period? Will it heal after treatment? Is there a possibility of recurrence? If treatment is not possible but the warts continue to grow, will it have any impact? If I have genital warts, do I have to choose a cesarean section?

Min, 20~29 year old female. Ask Date: 2016/06/10

Dr. Chen Jiansheng reply Urology

Hello Min: Human papillomavirus (HPV) infections often manifest when your immune system is weakened.
During pregnancy, the body's immune response changes, allowing the virus to reactivate.
If warts appear at the vaginal opening, they must be treated, as there have been reported cases where warts grew rapidly and caused vaginal obstruction.
Generally, a cesarean section is recommended to avoid transmitting the virus to the baby.
For treatment options, you should discuss this thoroughly with your obstetrician-gynecologist.

Managing genital warts during pregnancy can be a complex issue, as it involves balancing the health of the mother with the safety of the developing fetus. Genital warts, caused by the human papillomavirus (HPV), are a common sexually transmitted infection. During pregnancy, hormonal changes can sometimes lead to the growth of these warts, and they may become more prominent or numerous.

Treatment Options
1. Observation: In many cases, if the warts are not causing significant discomfort or complications, healthcare providers may recommend a watchful waiting approach. This is particularly true during the first and second trimesters of pregnancy. The rationale is that many warts may regress on their own postpartum, especially after the hormonal changes of pregnancy have resolved.

2. Topical Treatments: Some topical treatments, such as imiquimod (Aldara) or podofilox (Condylox), are generally avoided during pregnancy due to insufficient safety data. However, if the warts are extensive or symptomatic, a healthcare provider may consider these options with caution.

3. Cryotherapy: This involves freezing the warts with liquid nitrogen and is often considered safe during pregnancy. It can be effective in reducing the size of the warts and alleviating symptoms.

4. Surgical Options: In cases where warts are large or obstructive, surgical removal may be necessary. This can be done under local anesthesia, and it is typically safe during pregnancy. However, the timing of such procedures should be carefully considered, ideally postponed until after the first trimester if possible.

Concerns About Treatment
- Recurrence: Genital warts can recur after treatment, regardless of the method used. This is due to the nature of HPV, which can remain dormant in the body. While treatment can remove visible warts, it does not eliminate the virus itself.

- Impact on Pregnancy: Generally, genital warts do not pose a significant risk to the pregnancy or the fetus. However, if warts are large and obstructive, they could potentially complicate delivery. In such cases, a cesarean section may be recommended to avoid any risk of warts obstructing the birth canal.

- Delivery Method: The presence of genital warts does not automatically necessitate a cesarean delivery. The decision will depend on the size and location of the warts, as well as the overall health of the mother and baby. If the warts are small and not obstructive, vaginal delivery is usually possible.

Conclusion
If you are 13 weeks pregnant and have been diagnosed with genital warts, it is essential to discuss your treatment options with your healthcare provider. They can help you weigh the benefits and risks of various treatments and determine the best course of action based on your specific situation. Regular monitoring and open communication with your healthcare team will be crucial in managing your condition effectively while ensuring the health and safety of both you and your baby.
Remember, maintaining a healthy lifestyle, managing stress, and attending regular prenatal check-ups will also contribute positively to your overall well-being during pregnancy.
