Ear problems
Hello, I would like to ask you a question.
My child has been experiencing a lot of pressure and pain in the ear these past few days, and at night the pain becomes unbearable.
The pain radiates from the ear to the eye and head, all on the right side.
I am wondering if this could be otitis media, and if hospitalization is necessary for treatment of otitis media?

Dr. Ji Zhaoyun reply Otolaryngology
Hello: The issues you mentioned may be attributed to the following two possibilities: 1) Otitis Media: a) Acute Otitis Media: often occurs alongside upper respiratory infections (such as colds) and sinusitis, and in severe cases, there may be ear discharge.
b) Chronic Otitis Media with acute exacerbation: usually occurs when there is a pre-existing perforation in the eardrum, which can become inflamed due to unclean water or improper ear cleaning.
Treatment options include using ear drops along with oral medications (such as antibiotics and anti-inflammatory pain relievers) for mild cases; hospitalization is required for severe cases.
If hearing is affected, it is advisable to seek medical attention promptly.
2) Temporomandibular Joint Disorder (TMJ): This condition occurs in the joint located in front of the ear and often presents with ear pain, typically caused by poor bite habits.
It can be managed with medication and adjustments to biting habits.
Wishing you peace and good health.
Sincerely, Dr.
Chi Chao-Yun, Head of the ENT Department, Taichung Hospital, Ministry of Health and Welfare.

Hello! Thank you for reaching out with your concerns about ear pain. Based on your description of the symptoms—persistent pain in the ear, pressure, and discomfort radiating to the eye and head on the right side—it is indeed possible that you may be experiencing a middle ear infection, also known as otitis media.
Middle ear infections are quite common and can occur due to various reasons, including viral infections (like the common cold), bacterial infections, or even allergies. The middle ear is the space behind the eardrum that contains tiny bones that help transmit sound. When this area becomes inflamed or infected, it can lead to symptoms such as ear pain, a feeling of fullness or pressure in the ear, hearing loss, and sometimes fever.
In your case, the fact that the pain is severe enough to disrupt your sleep and radiates to other areas like the eye and head suggests that the infection may be significant. While not all cases of middle ear infections require hospitalization, some may need more intensive treatment, especially if there are complications or if the infection does not respond to initial treatments.
Typically, treatment for a middle ear infection may include:
1. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate pain and reduce fever.
2. Antibiotics: If the doctor suspects a bacterial infection, they may prescribe antibiotics. It’s important to complete the full course of antibiotics even if you start feeling better.
3. Warm Compresses: Applying a warm compress to the affected ear can provide some relief from pain.
4. Decongestants or Antihistamines: If allergies or nasal congestion are contributing to the ear infection, these medications may help.
5. Follow-Up Care: It’s crucial to follow up with your healthcare provider if symptoms persist or worsen. They may recommend further evaluation, such as a hearing test or imaging studies, to assess the extent of the infection.
In some cases, if the infection is severe or recurrent, or if there is fluid buildup that does not resolve, a procedure called tympanostomy may be considered. This involves placing tubes in the eardrum to help drain fluid and prevent future infections.
Given your symptoms, I recommend that you seek medical attention as soon as possible. An ear, nose, and throat (ENT) specialist can provide a thorough examination, possibly including an otoscopic examination to visualize the eardrum and middle ear. They can confirm the diagnosis and determine the best course of action for your specific situation.
In summary, while middle ear infections can often be managed with outpatient care, your symptoms warrant a professional evaluation to ensure appropriate treatment and to rule out any complications. Please take care of yourself, and I hope you find relief soon!
