External swelling and pain in the genital area?

A few days ago, I felt an itch in my private area before going to bed and scratched it.
The next day, it felt a bit painful, and I thought it was nothing serious.
However, a few days later, the pain increased and it became swollen.
Now, I can't even sleep at night due to the swelling and pain.
What should I do?

Ding Ding, 10~19 year old female. Ask Date: 2016/06/19

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, based on your description of symptoms, it is more likely to be folliculitis or skin abrasion.
It is recommended to keep the perineal area clean and dry, avoid tight-fitting clothing, minimize the use of panty liners, ensure adequate sleep, maintain a light diet, and avoid stress and prolonged sitting.
You may soak in a bath to relieve symptoms.
If symptoms persist, please consult a physician for diagnosis and treatment.
Thank you! Wishing you peace and health, Dr.
Zhang.

More Info

Experiencing pain and swelling in the external genital area can be distressing and uncomfortable. Based on your description, it seems that the initial itching led to scratching, which may have caused irritation or even an infection. Here’s a comprehensive understanding of your symptoms and what steps you can take to address them.

Possible Causes of External Genital Pain and Swelling
1. Irritation or Allergic Reaction: Scratching can lead to skin irritation or an allergic reaction to soaps, detergents, or personal care products. This can cause inflammation, redness, and swelling.

2. Infection: The symptoms you describe could indicate a bacterial or yeast infection. Vaginal yeast infections often cause itching and swelling, while bacterial infections can lead to pain and discomfort. Sexually transmitted infections (STIs) like herpes or gonorrhea can also present with similar symptoms.

3. Dermatitis: Conditions like contact dermatitis, which occurs when the skin reacts to irritants or allergens, can cause itching, redness, and swelling.

4. Folliculitis: This is an inflammation of the hair follicles, which can occur in the genital area and may result from shaving or friction.

5. Cysts or Abscesses: Sometimes, blocked glands can lead to cysts or abscesses, which can become painful and swollen.

Recommended Actions
1. Avoid Scratching: While it may be tempting, scratching can worsen the irritation and lead to further inflammation or infection.

2. Maintain Hygiene: Gently clean the area with mild soap and water. Avoid using harsh soaps or scented products that can exacerbate irritation.

3. Cold Compress: Applying a cold compress to the affected area can help reduce swelling and alleviate pain.

4. Over-the-Counter Treatments: Consider using over-the-counter hydrocortisone cream to reduce inflammation or antifungal creams if you suspect a yeast infection. However, be cautious and follow the instructions on the packaging.

5. Wear Loose Clothing: Opt for breathable, loose-fitting underwear and clothing to minimize friction and irritation.

6. Avoid Irritants: Stay away from potential irritants, including scented products, douches, and tight-fitting clothing.

7. Consult a Healthcare Provider: Given that your symptoms have worsened and are affecting your ability to sleep, it is crucial to see a healthcare provider. They can perform a physical examination and possibly run tests to determine the underlying cause of your symptoms. Early diagnosis and treatment are essential, especially if an infection is present.

When to Seek Immediate Care
- If you experience severe pain, fever, or discharge that has an unusual odor or color.

- If the swelling continues to worsen or if you notice any signs of systemic infection, such as chills or malaise.

- If you have any concerns about STIs, it is essential to get tested and treated promptly.

Conclusion
Pain and swelling in the external genital area can arise from various causes, ranging from irritation to infections. It is essential to take preventive measures and seek medical attention if symptoms persist or worsen. Your healthcare provider will be able to provide a tailored treatment plan based on the specific diagnosis, ensuring that you receive the appropriate care for your condition. Remember, early intervention can lead to better outcomes and relief from discomfort.
