Glans issues have been a source of concern for me?
Recently, I noticed that the urethral opening has become enlarged.
About a month ago, my glans got stuck to my underwear, and I felt pain at the connection point of the urethra.
Sometimes, when I retract the foreskin, the opening appears quite wide because it has enlarged.
Occasionally, some urethral tissue protrudes slightly, and I can push it back in.
I went to the Urology Department at Cheng Ching Hospital, and the doctor said there is no hypospadias (the doctor asked me to expose it for a distant view, which felt dismissive).
Can this be sutured? How much would it cost?

Dr. Sun Hongshun reply Urology
Hello Lenny: Although you have provided a detailed description, a physical examination is still necessary to confirm whether it is hypospadias.
If it is indeed hypospadias and requires surgery, it should be covered by health insurance.
If you still have questions, feel free to visit the urology clinic for a thorough examination by me.

When it comes to concerns regarding an enlarged urethral opening, particularly after an incident where the glans was caught in clothing, it is essential to understand the implications and potential treatments available. The symptoms you describe—pain, an enlarged urethral opening, and the appearance of urethral tissue protruding—can be indicative of several conditions, including trauma, inflammation, or even a potential urethral stricture.
Firstly, it is important to clarify that the urethral opening, or meatus, can sometimes appear enlarged due to trauma or irritation. In your case, the incident where the glans was caught could have caused some degree of trauma, leading to swelling or changes in the tissue. The fact that a healthcare professional has ruled out hypospadias (a congenital condition where the urethral opening is not located at the tip of the penis) is a positive sign, but it does not entirely eliminate the possibility of other issues arising from the trauma.
In terms of treatment options, if the enlargement of the urethral opening is causing functional issues, such as difficulty urinating or recurrent infections, surgical intervention may be considered. Urethral reconstruction or meatal plasty (a procedure to reshape the urethral opening) could be options, depending on the severity of the condition and the specific findings upon examination.
Regarding the cost of such procedures, it can vary widely based on several factors, including the healthcare facility, the complexity of the surgery, and whether you have insurance coverage. In the United States, for example, outpatient procedures can range from a few hundred to several thousand dollars. It is advisable to consult with your urologist about the specific costs involved, as they can provide a more accurate estimate based on your individual case and the facility where the procedure would be performed.
Additionally, if you are experiencing any discomfort or if the condition worsens, it is crucial to seek further evaluation. Sometimes, a second opinion can provide additional insights or alternative treatment options. If the urethral tissue is protruding, it may also be a sign of a condition known as urethral prolapse, which can occur in some individuals and may require specific treatment.
In the meantime, it is essential to maintain good hygiene and avoid any activities that may exacerbate the condition, such as tight clothing or vigorous activities that could lead to further trauma. Keeping the area clean and dry can help prevent infections, which can complicate the situation.
In summary, while an enlarged urethral opening can be concerning, it is important to approach the situation with a clear understanding of the potential causes and treatments. Consulting with a urologist who can provide a thorough examination and discuss the best course of action is crucial. If surgical intervention is necessary, discussing the costs and options available with your healthcare provider will help you make informed decisions about your health.
