Menstrual irregularities
Hello, doctor.
I haven't had my period for three months, and now it has been 22 days without finishing.
For the first one to two days, I had light bleeding in the morning, but it stopped by the afternoon.
It took until the fourth day for it to return to normal.
Although the flow is now very light, it still hasn't ended.
What could be the reason for this? Is this normal? Thank you.

Please consult an obstetrician-gynecologist for a detailed examination to identify the causes and provide targeted treatment for menstrual irregularities.
The two main causes are: (1) Poor ovarian function and endocrine disorders, and (2) Ineffective uterine contractions, which can lead to prolonged menstrual periods.

More Info
Irregular menstruation can be a source of concern for many women, and understanding its causes is crucial for addressing any underlying health issues. Based on your description, it seems that you are experiencing prolonged menstrual bleeding, which can be classified as abnormal uterine bleeding. This condition can arise from various factors, and it is essential to consider both physiological and pathological causes.
Possible Causes of Irregular Menstruation
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. Any disruption in this hormonal balance can lead to irregularities. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, and hyperprolactinemia can cause hormonal imbalances that affect menstruation.
2. Stress and Lifestyle Factors: High levels of stress, significant weight changes, excessive exercise, or changes in diet can impact hormonal levels and lead to irregular periods. Emotional stress can also affect the hypothalamus, which plays a key role in regulating the menstrual cycle.
3. Uterine Conditions: Structural abnormalities in the uterus, such as fibroids, polyps, or adenomyosis, can lead to heavy or prolonged menstrual bleeding. These conditions can cause the lining of the uterus to thicken, leading to heavier periods or bleeding between cycles.
4. Infections: Pelvic inflammatory disease (PID) or other infections of the reproductive system can cause irregular bleeding. These infections can lead to inflammation and changes in the menstrual cycle.
5. Medications: Certain medications, including hormonal contraceptives, anticoagulants, and some antidepressants, can affect menstrual regularity. If you have recently started or changed any medications, this could be a contributing factor.
6. Age-Related Changes: If you are approaching perimenopause (the transitional phase before menopause), you may experience irregular periods as your body undergoes hormonal changes.
When to Seek Medical Attention
Given that your menstrual cycle has been irregular for three months and you are experiencing prolonged bleeding, it is advisable to consult a healthcare provider. They may perform a thorough evaluation, which could include:
- Medical History Review: Discussing your menstrual history, any associated symptoms (such as pain or discomfort), and any medications you are taking.
- Physical Examination: A pelvic exam may be conducted to check for any abnormalities.
- Diagnostic Tests: Blood tests to check hormone levels, a pelvic ultrasound to visualize the uterus and ovaries, or a biopsy of the uterine lining if necessary.
Conclusion
While occasional irregularities in the menstrual cycle can be normal, prolonged bleeding or significant changes in your menstrual pattern warrant further investigation. It is essential to address any concerns with a healthcare professional who can provide personalized advice and treatment options based on your specific situation. Early intervention can help manage any underlying conditions and restore your menstrual health.
