Experiencing pain in the anus during bowel movements without any bleeding?

Hello, I started experiencing sharp pain in my anus when having a bowel movement since October last year.
Initially, the pain occurs only in the first half (about 5-6 seconds) and then subsides in the latter half.
There is no bleeding, and this has happened for several days in a month regardless of whether I have hard stools, soft stools, or diarrhea.
This situation has continued until now.
What could be the issue? Could it be a tumor?

Nicky, 30~39 year old female. Ask Date: 2016/07/05

Dr. Xu Kaixi reply Surgery

The sensation of anal pain, especially related to bowel movements, may be caused by abrasions or tears in the anal mucosa due to hemorrhoids.
It is recommended to visit a surgical outpatient clinic for examination and treatment.

Anal pain during bowel movements can be a distressing symptom, and it’s understandable to be concerned about the underlying causes. Based on your description, the pain occurs primarily at the beginning of the bowel movement and lasts for about 5-6 seconds, without any accompanying bleeding. This pattern can suggest several potential causes, but it is essential to approach this systematically.

One of the most common causes of anal pain during bowel movements is anal fissures. An anal fissure is a small tear in the lining of the anus, often caused by passing hard stools. This can lead to sharp pain during bowel movements, which may explain the brief, intense discomfort you experience. Fissures can occur regardless of whether the stool is hard or soft, especially if there is straining involved.
Another possibility is hemorrhoids, which are swollen veins in the rectal area. While hemorrhoids can cause pain, they are often associated with itching, swelling, and sometimes bleeding. If you are not experiencing any bleeding, this may be less likely, but it is still worth considering.

In some cases, anal pain can also be related to muscle spasms in the anal sphincter, known as anal sphincter spasms. These spasms can occur during bowel movements and can cause sharp pain.
While your concern about the possibility of a tumor is understandable, it is less common for tumors to present solely with pain during bowel movements without other symptoms such as significant changes in bowel habits, weight loss, or bleeding. However, it is crucial to rule out any serious conditions, especially if the pain persists or worsens.

Given that this issue has been ongoing since October, it is advisable to consult a healthcare professional for a thorough evaluation. A physical examination, including a visual inspection of the anal area, may be necessary to identify any fissures, hemorrhoids, or other abnormalities. In some cases, further diagnostic tests, such as an anoscopy, may be warranted to get a clearer view of the anal canal and rectum.

In the meantime, there are several self-care measures you can take to alleviate discomfort. Increasing dietary fiber intake can help soften stools and reduce straining during bowel movements. Staying well-hydrated is also essential. Over-the-counter stool softeners or fiber supplements can be beneficial. Additionally, warm sitz baths can provide relief by relaxing the anal muscles and soothing the area.

If you are diagnosed with an anal fissure, treatment may include topical medications to promote healing and relieve pain. In cases where conservative measures do not provide relief, further interventions may be necessary.

In summary, while there are several potential causes for your anal pain during bowel movements, it is essential to seek medical advice for a proper diagnosis and treatment plan. Early intervention can help prevent complications and improve your quality of life.
