Should I Continue Using Cream or Consider Surgery for Phimosis? - Urology

Should I proceed with the surgery or continue with the circumcision?

Hello doctor, I originally had a phimosis issue where I couldn't retract the foreskin before and after an erection.
Last week, after researching online, someone suggested applying Lidocaine to improve the condition.
I have been applying the medication for 7 days now, and while I can retract the foreskin to the glans when not erect, the skin appears thick like a scarf, but it has the color of new skin around the glans.
Initially, it was tight, but now it's less so, although it is still quite sensitive.
However, I still cannot retract it during an erection.
Should I try to retract the skin before an erection or apply the medication to the tight area after an erection? Will the skin around the glans stretch during an erection, or is surgery the only option?

Applying Rinderon ointment can help to separate adhered areas in children.
If the foreskin is too tight at your age, surgery may still be necessary.

When dealing with phimosis, a condition where the foreskin cannot be easily retracted over the glans (head) of the penis, it is important to consider both non-surgical and surgical options based on the severity of the condition and the response to treatment. In your case, you've been using a topical steroid cream (likely a corticosteroid) for about a week, and you've noticed some improvement, such as the foreskin being able to retract partially when not erect. However, it remains sensitive and tight during an erection.

Topical corticosteroids like the one you are using can help soften the skin and reduce inflammation, making it easier to retract the foreskin over time. This treatment is often recommended as a first-line approach for mild to moderate phimosis, especially in cases where there is no significant scarring or other complications. The goal is to gradually stretch the foreskin and improve its elasticity.

Here are some important considerations regarding your situation:
1. Continued Use of Cream: Since you have seen some improvement with the cream, it is advisable to continue using it as directed by your healthcare provider. Apply the cream to the tight areas of the foreskin, both when flaccid and erect if possible, to help with stretching. It may take several weeks of consistent use to achieve the desired results.

2. Timing of Application: You can apply the cream both before and after an erection. However, applying it after an erection may be more effective as the skin is already stretched, allowing the cream to penetrate better. Be gentle when retracting the foreskin to avoid injury.

3. Sensitivity and Thickness of Skin: The sensitivity you are experiencing is common, especially if the skin has been tight for a long time. As you continue treatment, the skin should gradually become less sensitive and more pliable. If the skin appears thickened, this may also improve with continued use of the steroid cream.

4. Surgical Consideration: If after a few weeks of treatment you do not see significant improvement, or if the phimosis is causing pain, recurrent infections, or other complications, surgical options such as circumcision or preputioplasty may be necessary. These procedures can provide a more definitive solution to phimosis.

5. Consultation with a Specialist: It is essential to keep in touch with your healthcare provider throughout this process. They can monitor your progress and make recommendations based on your specific situation. If you notice any adverse reactions to the cream or if your symptoms worsen, seek medical advice promptly.

6. Patience and Compliance: Treatment for phimosis can take time, and it is important to be patient and compliant with the treatment regimen. Regular follow-ups with your healthcare provider can help ensure that you are on the right track.

In summary, continue using the topical steroid cream as directed, apply it at appropriate times, and monitor your progress. If you do not see improvement or if the condition worsens, consult your healthcare provider to discuss potential surgical options. Remember, every case is unique, and your healthcare provider is the best resource for personalized advice.
