Abnormal discharge with a slight presence of blood?

Hello doctor, my last menstrual cycle started around June 30.
After having sexual intercourse with my boyfriend on July 16, I started experiencing slight bleeding the next day, which resembled the color of the beginning of my period.
For the past week, I have had brownish discharge, but the amount has been minimal.
Could this be related to not having completely cleared my previous menstrual cycle? I have experienced similar situations before, but they usually stopped after about 3 to 4 days.
However, this time it has persisted for nearly a week.
Today, I also started feeling some discomfort in my abdomen, and I have a burning sensation when I urinate.
After taking a shower, the discharge changed to something resembling cervical mucus mixed with light brown.
What could this possibly indicate?

Niming, 20~29 year old female. Ask Date: 2016/07/24

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors that can affect the menstrual cycle or cause intermenstrual bleeding are numerous, including environment, lifestyle, stress, emotional instability, diet, age, lesions in the uterus or cervix, and pregnancy, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health.

Understanding abnormal discharge with blood can be concerning, especially when accompanied by other symptoms. Based on your description, you experienced light bleeding after sexual intercourse, followed by a week of brown discharge, and now you are feeling some abdominal discomfort and a burning sensation when urinating. Let's break down the possible causes and concerns related to your symptoms.

1. Implantation Bleeding: If you are sexually active and there is a possibility of pregnancy, the light bleeding you experienced could be implantation bleeding. This occurs when a fertilized egg attaches to the lining of the uterus, typically around the time your period is due. However, this is usually lighter than a regular period and may be accompanied by brown discharge.

2. Hormonal Fluctuations: Irregular bleeding can also be attributed to hormonal changes in your menstrual cycle. Stress, changes in weight, or hormonal contraceptives can lead to spotting or brown discharge. If your menstrual cycle has been irregular, this could be a contributing factor.

3. Infection: The burning sensation during urination and the change in discharge could indicate a urinary tract infection (UTI) or a sexually transmitted infection (STI). Infections can cause abnormal discharge, discomfort, and changes in menstrual patterns. It's essential to get tested for STIs if you have had unprotected sex.

4. Cervical or Uterine Issues: Conditions such as cervical polyps, fibroids, or even cervical inflammation can lead to abnormal bleeding and discharge. These conditions may cause discomfort and should be evaluated by a healthcare provider.

5. Retained Products of Conception: If you have had a recent miscarriage or abortion, retained tissue can cause prolonged bleeding and discharge. This would typically be accompanied by other symptoms such as severe cramping or fever.

6. Endometriosis or Pelvic Inflammatory Disease (PID): Endometriosis can cause abnormal bleeding and pelvic pain, while PID can lead to similar symptoms, including abnormal discharge and discomfort. Both conditions require medical evaluation and management.

Given that your symptoms have persisted for over a week and are accompanied by discomfort, it is crucial to seek medical attention. A healthcare provider can perform a physical examination, possibly including a pelvic exam, and may recommend tests such as a urine analysis, blood tests, or imaging studies to determine the underlying cause of your symptoms.

In the meantime, it is advisable to monitor your symptoms closely. If you experience severe pain, fever, or heavy bleeding, seek immediate medical attention. Avoid self-diagnosing or self-treating, especially if you suspect an infection or other serious condition.

In summary, while there are several potential causes for your symptoms, it is essential to consult with a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and ensure your reproductive health is maintained.
